Congenital Dilatation of Extrahepatic Bile Ducts in Children. Experience in the Central Hospital of Hue, Vietnam

Abstract

Background/Purpose: The authors present a study of a series of cases in children with congenital dilatation of the extrahepatic bile ducts (CDEBD). Methods: Between November 1998 and October 2002, 38 children aged between 50 days and 15 years suffering from CDEBD, admitted to the Central Hospital in Hue, Vietnam, were treated surgically with a minimum follow-up of one year. Results: Diagnosis was based only on ultrasonography which was 100 % accurate. According to Miyano's classification, 26 cases presented as cystic dilatation of the main bile duct (MBD) associated with dilatation of the intrahepatic bile ducts, while in the other 12 cases the dilatation of the MBD was of the fusiform type. The surgical treatment of choice was extensive excision of the dilatated extrahepatic bile ducts and biliary drainage according to the Roux-en-Y method in 36/38 patients. The other two patients underwent surgery for internal biliary drainage without removing the cysts. During follow-up we observed one case of pancreatitis out of the 36 patients who underwent excision of the dilatated extrahepatic bile ducts, while the two patients who had internal biliary drainage without removal of the cysts suffered from numerous attacks of cholangitis. Conclusions: The reflux of bile in the dilated biliary tree plays an important role in the etiopathogenesis of CDEBD. Excision of the extrahepatic bile ducts and internal biliary drainage by Roux-en-Y has proved a satisfactory surgical method.

Les auteurs présentent une étude d'une série de cas d'enfants avec une dilatation congénitale des voies biliaires extra-hépatiques (CDEBD). Méthode: De Novembre 1998 à Octobre 2002, 38 enfants âgés de 50 jours à 15 ans souffrant d'une CDEBD sont admis à l'Hôpital central de Hue au Vietnam et traités chirurgicalement avec un suivi minimum de un an. Résultat: Le diagnostic était basé seulement sur l'ultrasonographie qui était efficace à 100 %. Conformément à la classification de Miyano, 26 cas présentaient une dilatation de la voie biliaire principale (MBD) associée à une dilatation des voies biliaires intra-hépatiques alors que les 12 autres cas, la dilatation de la MBD était fusiforme. Le traitement chirurgical de choix est une excision extensive de la voie biliaire dilatée et un drainage biliaire réalisé par une anse en «Y» à la Roux chez 36 patients. Les deux autres patients ont subi un drainage biliaire interne sans exérèse des kystes. Nous avons observé un cas de pancréatite parmi les 36 patients qui ont subi une excision de la dilatation de la voie biliaire, alors que les deux patients qui avaient un drainage biliaire interne sans ablation du kyste souffraient de nombreux épisodes de cholangite. Conclusion: Le reflux de bile dans l'arbre biliaire joue un rôle important dans la pathogénie de la CDEBD. L'excision de la voie biliaire extra-hépatique et de drainage par une anse en «Y» a démontré être une méthode satisfaisante.

Resumen

Objetivo: Se presenta una serie de niños con Dilatación Congénita de la vía biliar extrahepática (CDEBD). Métodos: Entre Noviembre de 1998 y Octubre de 2002, fueron tratados quirúrgicamente en el Hospital Central de Hue, Vietnam, 38 niños de entre 50 días y 15 años con CDEBD y fueron seguidos durante un mínimo de 1 año. Resultados: El diagnóstico se basó unicamente en la ecografía que fue certera en el 100 % de los casos. Según la clasificación de Miyano, 26 casos tenían dilatación quística del colédoco (MBD) asociada con dilatación de los conductos intrahepáticos mientras que en los restantes 12 casos la dilatación del MBD era fusiforme. El tratamiento quirúrgico de elección fue la extirpación amplia de la vía biliar dilatada y el drenaje biliar con asa en Y de Roux en 36/38 patients. En los otros dos se realizó drenaje biliar interno sin extirpación del quiste. En el seguimiento observamos un caso de pancreatitis entre los 36 pacientes en quienes se extirpó la vía biliar dilatada mientras que los dos con derivación sin extirpación sufrieron numerosas crisis de colangitis. Conclusiones: El reflujo de bilis al árbol biliar dilatado juega un papel importante en la etiopatogenia de la CDEBD. La extirpación de los conductos extrahepáticos y la derivación interna en Y de Roux son un método satisfactorio de tratamiento.

Zusammenfassung

Zielsetzung: Die Autoren stellen eine Serie von Kindern mit angeborener Dilatation der extrahepatischen Gallenwege (CDEBD) vor. Methodik: Zwischen November 1998 und Oktober 2002 wurden 38 Kinder im Alter von 50 Tagen bis 15 Jahren wegen einer CDEBD im Zentralkrankenhaus in Hue, Vietnam, chirurgisch behandelt und mindestens 1 Jahr lang nachuntersucht. Ergebnisse: Die Diagnose wurde mittels Ultraschall gestellt und war in 100 % korrekt. Nach der Klassifikation von Miyano wiesen 26 Fälle eine zystische Dilatation des Hauptgallenganges mit Erweiterung der intrahepatischen Gallenwege, 12 Kinder eine fusiforme Dilatation des Hauptgallenganges auf. Die chirurgische Therapie bestand bei 36 von 38 Kindern in einer ausgedehnten Resektion des dilatierten Gallenganges mit einer Roux-Y-Ableitung. Bei zwei weiteren Patienten wurde eine interne Drainage ohne Resektion der Zyste vorgenommen. Im Nachuntersuchungszeitraum wurde bei einem Patienten nach Resektion der Dilatation eine Pankreatitis beobachtet, während die beiden Kinder mit interner Drainage ohne Resektion unter rezidivierenden Cholangitiden litten. Schlussfolgerungen: Der Reflux von Pankreassaft und Galle in die erweiterten Gallenwege spielt eine wichtige Rolle in der Ätiopathogenese der CDEBD. Die Exzision der extrahepatischen Gallenwege mit Roux-Y-Anastomose führen zu befriedigenden Ergebnissen.
