Wie symmetrisch oder asymmetrisch ist der normale bzw. physiologische Gang in Abhängigkeit vom Alter?

Zusammenfassung

Fragestellung: Eine wichtige Voraussetzung für die Beurteilung pathologischer Gangmuster ist die Quantifizierung der Symmetrieverhältnisse beim physiologischen Gang. Ziel der Studie ist es, die Symmetrie von Parametern der Bodenreaktionskraft komponenten- und altersspezifisch zu untersuchen. Material und Methode: Die Senioren- (SG = 30: 59,5 ± 9,1 J.) und die Juniorenstichprobe (JG = 30: 25,1 ± 1,9 J.) unterscheiden sich im Alter hochsignifikant. Die Probanden führten 10 - 12 Gehversuche auf der Gangbahn mit zwei integrierten 3D-Kraftmessplattformen aus. Von insgesamt 24 Kraft-Zeit-Parametern wurden die Seitigkeitsverhältnisse, die Symmetrieindizes und die altersspezifischen Tendenzen bestimmt und statistisch abgeprüft. Ergebnisse: Aufgrund signifikanter Varianzen unterscheiden sich die Ganggeschwindigkeiten nicht (SG: 1,40 ± 0,125 m/s; JG: 1,46 ± 0,23 m/s). Die Dauer der Stütz- und Doppelstützphasen ist dagegen bei den Senioren signifikant länger. In den Kraft-Zeit-Parametern treten signifikante Differenzen zwischen den Stützphasen bei den Junioren häufiger auf. Die mathematisch „ideale” Symmetrie (SI = 0) gibt es bei keinem Probanden und Parameter. Die Indizes sind komponentenspezifisch einzuordnen: Je kleiner die Kraftkomponente gegenüber der Resultierenden, umso größer die Asymmetrie. Vgl. SI: Fz < 7 %; Fy < 10 %; Fx < 22 %. Altersbedingte Veränderungen in den Symmetrieverhältnissen wurden nicht ermittelt. Schlussfolgerung: Für die wissenschaftliche Bearbeitung der Problematik kann die Anzahl der Parameter reduziert und für klinische Studien standardisiert werden. Funktionelle Parameter sind für die ganganalytische Verlaufskontrolle zu empfehlen.

Abstracts

Purpose: The age-specific and the parameter-specific quantification of asymmetries in normal human gait is essential for the assessment of pathological gait. The purpose of this study is to investigate symmetries of force and time parameters considering age and different parameters. Material and methods: There are highly significant differences in the mean values of age between the senior group (sg = 30: 59.5 ± 9.1 ys) and the junior group (jg = 30: 25.1 ± 1.9 ys). 24 force and time parameters were collected while subjects performed 10 - 12 gait trials on the walkway with two embedded 3D-force platforms. A symmetry index and age-specific tendencies had been calculated for all parameters before statistical analysis could be applied. Results: Due to significant variances gait velocities are not statistically different in the mean values between the groups (sg: 1.40 ± 0.125 m/s; jg: 1.46 ± 0.23 m/s). However, stance and double support time last significantly longer in the elderly group. The younger group showed significant differences between left and right in force and time parameters more often. None of the subjects exhibited the mathematical „ideal” gait symmetry (SI = 0) for the measured parameters. Indices are to be investigated and arranged according to the different force components: The lower the force component, the higher the asymmetry (Fz < 7 %; Fy < 10 %; Fx < 22 %). Age-related changes in symmetry indices could not be found. Conclusions: For further investigation of symmetries the number of parameters could be reduced. In clinical studies parameters should also be standardised. Functional parameters can be recommended for evaluating the functional outcome of interventions.
