Zusammenfassung
Anamnese und klinischer Befund: Eine 27-jährige Patientin stellte sich wegen eines wandernden Fremdkörpergefühls in der Wangenschleimhaut wiederholt in der Arztpraxis vor. Die Inspektion blieb jedesmal ohne erkennbaren klinischen Befund. Bei einer Wiedervorstellung präsentierte die Patientin ein 4 cm langes filiformes Gebilde, dass sie nach Manipulation mit der Zahnbürste aus der Mundhöhle extrahiert habe.
Untersuchung: Mikroskopisch handelte es sich um ein gravides Nematoden-Weibchen der Gattung Gongylonema.
Diagnose: Gongylonema pulchrum-Infektion.
Therapie und Verlauf: Im Verlauf wurde ein zweiter Wurm in der kontralateralen Wangenschleimhaut dargestellt und entfernt. Ohne weitere therapeutische Intervention war die Patientin anschließend dauerhaft beschwerdefrei.
Folgerungen: Auch eine scheinbar sehr ungewöhnliche Symptomatik, die primär an einen Dermatozoenwahn denken lassen könnte, sollte dezidiert abgeklärt werden. Gegebenenfalls ist zum Ausschluss einer möglichen Parasitose eine entsprechende Fachabteilung einzubeziehen.
Summary
History: A 27-year-old woman repeatedly attended an outpatient clinic and reported sensations of movement within the buccal mucosa. Various local inspections were without any obvious abnormal findings. A few days later the patient brought a filiform structure, which she had extracted from the affected mucosa by manipulation with her toothbruch.
Investigation: Microscopy indicated a gravid female nematode of the genus Gonglyonema.
Diagnosis: Gonglyonem pulchrum infection.
Treatment and course: Careful inspection revealed a second worm at the other side of the oral cavity, which was extracted by local incision. After removal of the second worm the patient remained free of symptoms without any further therapy.
Conclusion: Even obviously very unusual symptoms, which might be indicative of a dermatozoic delusion, should be carfully clarified. In some cases it may be appropriate to consult a specialized institutions or laboratory to rule out a possible parasitic infections.
