Abstract
Aim: This study aimed to investigate antioxidant capacity by using a novel automated method in children with acute appendicitis. Methods: Blood samples were obtained from consecutive patients with acute appendicitis (appendicitis group, n = 12) and acute abdominal pain due to non surgical disease (non-appendicitis group, n = 11), and from patients with inguinal hernia (healthy group, n = 12) as the control group. At admission, total antioxidant capacity (TAC) levels of plasma were evaluated in all patients by a method recently developed by Erel. Four other major individual plasma antioxidant components, the levels of total protein, albumin, uric acid and bilirubin, were also evaluated. Total antioxidant capacity in patients with acute appendicitis was statistically compared with the two other groups. Results: While the TAC level in the appendicitis group was significantly greater than in the non-appendicitis group, no significant difference was found in healthy groups (p < 0.05, p > 0.05, 1.94 ± 0.38, 1.40 ± 0.36, and 1.99 ± 0.35 respectively). Individual components of total antioxidant capacity, i.e. total protein, albumin, uric acid and bilirubin concentrations, were also higher in the patients with acute appendicitis than those of the other two control groups. Conclusions: Our data show that children with acute appendicitis do not have deficient blood plasma antioxidant capacity. These results provide evidence that acute appendicitis results in more induction of antioxidative response than non-surgical diseases.

But: Apprécier les capacités anti-oxydantes par l'utilisation de nouvelles techniques chez les enfants présentant une appendicite aiguë. Méthode: Des prélèvements sanguins étaient obtenus de patients avec une appendicite aiguë (groupe appendicite n = 12) et des douleurs abdominales dues à une situation non chirurgicale (groupe non appendicite n = 11), le patient avec une hernie inguinale (groupe sain n = 12) comme groupe contrôle. A l'admission, chez tous les patients, le taux de capacité totale anti-oxydante (TAC) du plasma étaient évalués par une méthode développée récemment par Erel. Quatre autres composants anti-oxydants du plasma étaient évalués: taux total de protéines, d'albumine, d'acide urique et de bilirubine. La capacité anti-oxydante totale des patients avec une appendicite aiguë était statistiquement comparée aux deux groupes. Résultats: Alors que le taux de TAC dans le groupe appendicite était significativement plus important que dans le groupe non appendicite, il n'y avait pas de différence significative avec le groupe sain (p < 0,05, p > 0,05, 1,94 ± 0,38, 1,40 ± 0,36, et 1,99 ± 0,35 respectivement). Les composants de la capacité anti-oxydante totale sont la protéine totale, l'albumine, l'acide urique et la bilirubine étaient aussi plus élevés chez les patients avec une appendicite aiguë que ceux des deux autres groupes de contrôle. Conclusion: Nos résultats montrent que les enfants avec une appendicite aiguë n'ont pas un déficit des capacités anti-oxydantes du plasma. Ces résultats montrent que l'appendicite aiguë entraîne une induction de la réponse anti-oxydante plus importante que les infections non chirurgicales.
Resumen
Objetivo: Investigar la capacidad antioxidante usando un nuevo método automatizado en niños con apendicitis aguda. Métodos: Se obtuvieron muestras sanguíneas de paciente consecutivos con apendicitis aguda (grupo apendicitis, n = 12) y dolor abdominal debido a enfermedad no quirúrgica (grupo no apendicitis, n = 1) y de pacientes con hernia inguinal (grupo sanos, n = 12) como controles. A la admisión se evaluó en todos los pacientes la capacidad total antioxidante (TAC) del plasma por el método recientemente desarrollado por Erel. Se determinaron también otros 4 componentes antioxidantes individuales del plasma, proteínas totales, albúmina, ácido úrico y bilirrubina. La capacidad antioxidante total en pacientes con apendicitis aguda fue comparada estadísticamente con la de los otros dos grupos. Resultados: Mientras la TAC en el grupo de apendicitis era significativamente mayor que en el no apendicitis, no se encontraron diferencias significativas con el grupo control (p < 0,05, p > 0,05, 1,94 ± 0,38, 1,40 ± 0,36, y 1,99 ± 0,35 respectivamente). Los componente individuales de la capacidad total antioxidante que son las proteínas totales, la albúmina, el ácido úrico y la bilirrubina eran también más altos en pacientes con apendicitis aguda que en el grupo control. Conclusiones: Nuestros datos muestran que los niños con apendicitis aguda no tienen deficiente capacidad antioxidante en el plasma. Estos resultados evidencian que la apendicitis aguda produce más inducción de respuesta antioxidativa que las enfermedades no quirúrgicas.
Zusammenfassung
Zielsetzung: In der vorliegenden Untersuchung wird die antioxidative Kapazität (TAC) der akuten Appendizitis im Kindesalter mittels einer neuen automatisierten Methode untersucht. Methodik: Prospektiv wurden Blutproben von Patienten mit akuter Appendizitis (n = 12) und akuten Bauchschmerzen ohne chirurgischer Relevanz (n = 11) gemessen, darüber hinaus von 12 Patienten mit Leistenhernien als Kontrollgruppe. Bei der Aufnahme wurden die TAC-Spiegel im Plasma nach der kürzlich von Erel entwickelten Methode bestimmt. Vier weitere wichtige, antioxidative Plasma-Komponenten wie der Gesamteiweißspiegel, die Albumin-Fraktion, die Harnsäure und das Gesamtbilirubin wurden ebenfalls untersucht. Die TAC von Patienten mit akuter Appendizitis wurde mit der TAC der beiden anderen Gruppen statistisch verglichen. Ergebnisse: Die TAC-Spiegel der Patienten mit akuter Appendizitis waren signifikant höher als die der nicht Appendizitis-Gruppe, jedoch nicht signifikant gegenüber der Leistenhernien-Gruppe (p < 0,05, p > 0,05, 1,94 ± 0,38, 1,40 ± 0,36 und 1,99 ± 0,35). Die Einzelkomponenten der TAC d. h. das Gesamteiweiß, die Albuminfraktion, die Harnsäure und das Gesamtbilirubin waren bei Patienten mit akuter Appendizitis ebenfalls höher als in den beiden anderen Gruppen. Schlussfolgerungen: Unsere Daten zeigen, dass Patienten mit einer akuten Appendizitis kein Defizit an antioxidativer Kapazität entwickeln. Die Ergebnisse weisen außerdem darauf hin, dass eine akute Appendizitis in stärkerem Ausmaß eine antioxidative Antwort auslöst als nicht chirurgische Erkrankungen.
