Die Deziduose - eine seltene lebensbedrohliche Komplikation in der Schwangerschaft

Zusammenfassung

Die Deziduose ist eine schwangerschaftsassoziierte Stromareaktion, die alle Gewebearten betreffen kann. In seltenen Fällen und bei entsprechendem Ausmaß kann sie zu massiven Blutungskomplikationen in der Schwangerschaft führen. Wir stellen den Fall einer Primgraviden vor, die in der 26. SSW primär unter dem Verdacht eines nekrotisierenden Seitenwandmyoms aufgenommen und 14 Tage später unter dem Bild einer intraabdominalen Massenblutung laparotomiert werden musste. Die histologische Differenzialdiagnostik wird ausführlich diskutiert.

Abstract

Deciduosis is a stromal reaction which is pregnancy associated and may concern all types of intraabdominal tissues. In rare circumstances and provided a certain extent it may lead to massive intraabdominal haemorrhage. We present the case of a primigravida who initially was admitted in the 26th week of gestation with the diagnosis of necrotising lateral myoma. A fortnight later she required emergency laparotomy due to extensive intraabdominal bleeding. We intensively describe and discuss the histological differential diagnostic procedures.
