Zusammenfassung
Ziel: Ermittlung der Erfolgs- und Komplikationsrate der CT-gesteuerten Markierung pulmonaler Rundherde vor videoassistierter thorakoskopischer chirurgischer Resektion (VATS). Material und Methoden: Bei 30 Patienten (20 Männer, mittleres Alter 57,6 ± 15,5 Jahre, bei 22 Patienten maligne Vorerkrankung) wurde ein Lungenherd (mittlerer Durchmesser 9 ± 5 mm, mittlerer Abstand zur Pleura 7 ± 5 mm) vor VATS CT-gesteuert mit einer Spirale markiert. Ziel war ein Spirale-Herd-Abstand von ≤ 10 mm. Ergebnisse: Die technische Erfolgsrate betrug 86,7 %. In 90 % wurde ein Abstand ≤ 10 mm zum Lungenherd erreicht. 70 % der Eingriffe wurden im Rahmen der VATS beendet, von denen in 81 % der Herd thorakoskopisch nicht sichtbar war. Bei 4 Patienten musste wegen folgender Probleme zur Thorakotomie gewechselt werden: 2 × präoperative Drahtdislokation, 1 × großer Abstand der Spirale zum Lungenherd, 1 × ungünstige Richtung der Markierung. Eine definitive histologische Sicherung wurde bei allen Patienten erreicht: In 70 % handelte es sich um maligne, in 30 % um benigne Herde. Im Rahmen der Markierung traten keine interventionsbedürftigen Komplikationen auf. Schlussfolgerung: Die CT-gesteuerte Lungenmarkierung ist die Voraussetzung für eine videoassistierte thorakoskopische chirurgische Resektion (VATS) von Lungenrundherden, die pleural nicht erkennbar sind. Die technische Erfolgsrate ist hoch bei geringer Komplikationsrate.
Abstract
Purpose: To assess the success and complication rate of the CT-guided marking of pulmonary nodules for video-assisted thoracoscopic surgery (VATS). Materials and Methods: Pulmonary nodules (mean diameter 9 ± 5 mm, mean pleural distance 7 ± 5 mm) were marked with a coil wire in 30 patients (20 males, mean age 57.6 ± 15.5 years, 22 patients with a history of malignancy). The intended coil-nodule distance was ≤ 10 mm. Results: 81 % of nodules were not visible by thoracoscopy. The technical success rate of CT-guided marking was 86.7 %. The projected nodule-coil distance was achieved in 90 % of cases. The procedure had to be changed from thoracoscopy to thoracotomy in 4 patients due to coil wire marking problems: 2 × coil displacement, 1 × coil-nodule distance > 10 mm, unfavorable direction of wire. Histology was determined in all patients (70 % malignant, 30 % benign). Complications requiring therapy were not observed. Conclusions: The CT-guided marking of pulmonary nodules is a precondition for VATS if the nodule does not involve the visceral pleura in the majority of cases. The success rate is high with a low complication rate.
