Die Hyperthermie im Gesamtkonzept der Behandlung des Bauchfellkrebs

Zusammenfassung

Der Befall des Peritoneums, auch als Peritonealkarzinose bezeichnet, stellt eine sehr aggressive Tumorart dar. Diese Ausbreitungsform verschiedener Primärtumore, vornehmlich aus dem Bereich des Gastrointestinaltrakts oder vom Bauchfell selbst ausgehend, wird zumeist in einem späten Stadium diagnostiziert und ist häufig verbunden mit einem raschen körperlichen Verfall. Dies bedingt entsprechend die ausgesprochen schlechte Prognose. Während des letzten Jahrzehnts konnten wesentliche Fortschritte gemacht werden in der Behandlung dieser Erkrankung durch ein neuartiges Verfahren, bestehend aus einer zytoreduktiven Chirurgie in Kombination mit einer intraperitonealen Chemotherapie unter hyperthermen Bedingungen. Damit wurde es möglich, Langzeitüberleben zu erreichen bei einer Erkrankungsform, die lange Zeit als inkurabel gehalten wurde.

Summary

Peritoneal carcinosis is a very aggressive type of cancer combined with late diagnosis, early onset of clinical detoriation and short survival time. Peritoneal carcinosis has traditionally been regarded as untreatable. During the last decade, however, results of cytoreductive procedures associated with intraperitoneal hyperthermic chemotherapy have lead to a paradigm change in a subgroup of patients with isolated peritoneal disease, in whom long term survival can be achieved.
