Neuere Entwicklungen zur Indikationsstellung einer Hochdrucktherapie sowie zur Frage einer Hierarchie antihypertensiver Substanzen

Zusammenfassung

Die arterielle Hypertonie hat eine entscheidende Bedeutung für die Entwicklung kardiovaskulärer Erkrankungen und ist in den Industrienationen eine wichtige Ursache für vorzeitige Morbidität und Mortalität. In den letzten Jahren hat sich die Indikation zur Hochdrucktherapie dahingehend geändert, dass der Blutdruck nicht mehr isoliert beurteilt wird, sondern in ein kardiovaskuläres Gesamt-Risikokonzept eingebettet wird. Es geht dabei nicht mehr primär darum, den Blutdruck zu senken, sondern darum, durch die antihypertensive Therapie das individuelle kardiovaskuläre Gesamtrisiko zu senken bzw. zu entscheiden, ob eine solche Senkung überhaupt notwendig und/oder gewünscht ist. Diuretika, Beta-Blocker, ACE-Hemmer, Angiotensinrezeptorblocker und Kalziumantagonisten senken die kardiovaskuläre Morbidität und Mortalität und werden von den internationalen Leitlinien als Medikamente der ersten Wahl zur Therapie der arteriellen Hypertonie bezeichnet. Die Effektgröße bezüglich der Senkung der kardiovaskulären Morbidität und Mortalität zeigt nur geringfügige Unterschiede zwischen den fünf Substanzklassen. Innerhalb der Substanzklassen besteht jedoch Inhomogenität, bezüglich des klinischen Nutzens bei der Minderung kardiovaskulärer Morbidität und Mortalität der einzelnen Wirkstoffe.

Abstract

High blood pressure is one of the major causes for cardiovascular diseases in industrial nations. The indication to treat high blood pressure has changed. Antihypertensive therapy is given to lower the individual cardiovascular global risk rather than reducing the blood pressure itself. For diuretics, β-blockers, angiotensin-converting-enzymes, angiotensin-receptor-blockers and calcium-channel-blockers it is shown that cardiovascular morbidity and mortality can be reduced and these drug-groups are recommended in current guidelines as first-line therapy in hypertension. There is no significant difference in between these five drug-groups in terms of reducing cardiovascular morbidity and mortality. Meanwhile substances within one drug-group differ in terms of cardiovascular benefit.
