Zusammenfassung
Pflanzliche Produkte werden in den westlichen Ländern immer häufiger eingenommen, dies sollte auch bei unklaren Fällen im klinischen Alltag berücksichtigt werden. Wir beschreiben den Fall eines 70-jährigen Patienten, der wegen unklarer und akut aufgetretener Xerostomie zugewiesen wurde. Die sorgfältige und vollständige klinische Untersuchung ergab ein anticholinerges Syndrom mit vielfältigen Befunden, und erst die explorative Detailanamnese deckte die initial verschwiegene Lupineningestion als Ursache des Syndroms auf. Lupinen werden in der Lebensmittelbranche unter anderem durch ihren hohen Proteingehalt und ihr geringes Allergenpotenzial hervorgehoben, medizinisch sind sie jedoch insbesondere wegen Anaphylaxiegefahr und Toxizität nicht unbedenklich.
Abstract
Formerly uncommon agricultural vegetables are being used more often in Western cuisine nowadays, a fact clinicians should consider when confronted with etiologically unclear cases. We describe the case of a 70 year old patient who was admitted due to acute and initially unexplainable xerostomia. A meticulous and complete examination revealed an anticholinergic syndrome with several diagnostic findings, but only the detailed exploration of the patient's anamnesis disclosed the ingestion of lupines as the cause for the syndrome. Lupines are advertised by food producers as healthy because of their high percentage in protein and low allergenic potential. From a medical point of view, however, they pose some risks regarding anaphylaxis and toxicity.
