Indikationen zur Liquordiagnostik in der Psychiatrie

Zusammenfassung

Anliegen Die Indikation zur Liquoruntersuchung in der Psychiatrie nach Literaturempfehlungen, klinischen Erfahrungsberichten und neuen Forschungsergebnissen darzustellen. Ergebnis Dringend ist Liquoruntersuchung bei Verdacht auf entzündliche ZNS-Erkrankungen aufgrund klinisch-anamnestischer Daten, CCT oder MRI-Befund. Bei neurodegenerativen Erkrankungen, affektiven und schizophrenen Psychosen werden Proteinabweichungen und immuninflammatorische Veränderungen berichtet. Schlussfolgerung Die Liquordiagnostik hat in der psychiatrischen Klinik eine begrenzte Indikation, für die Forschung gewinnt sie an Bedeutung.

Abstract

Aim and method Cerebrospinal fluid investigation in psychiatry is reviewed based on literature, clinical case reports and new research results. Results CSF investigation is generally recommended or even urgent if an inflammatory CNS disorder has to be considered from clinical symptoms (neurological abnormalities accompanying various psychiatric syndromes) from anamnestic data, or from imaging findings (CCT and/or MRI, MRI being most sensitive). In clinical research, CSF investigation is used to better understand the etiopathogenesis of affective and schizophrenic spectrum psychoses or neurodegenerative disorders including Alzheimer's. An increasing number of studies report protein or immunological or inflammatory abnormalities within CSF spaces (which may be primary or secondary) in schizophrenic and affective disorders. Summing up CSF investigation is urgently indicated in a relatively limited number of psychiatric patients but gains increasing importance in clinical research.
