Zusammenfassung
Anliegen Retrospektive Analyse des Einflusses der antidepressiven Medikation auf den Krankheitsverlauf von Patienten mit einer depressiven Störung (n = 312). Besonderes Interesse galt dem Einfluss auf Verweildauer und stationäre Wiederaufnahmerate. Methode Mittels BADO-Daten wurden antidepressive Mono- und Kombinationstherapie, Verweildauer und Verlaufsparameter erfasst. Ergebnisse Die durchschnittliche Verweildauer betrug 54 Tage, die Zeit bis zur 1. Wiederaufnahme 112 Tage. 26 % der Patienten erhielten eine Monotherapie mit einem modernen Antidepressivum (AD), 2 % mit einem konventionellen. 54 % erhielten eine Kombination aus zwei modernen AD und 18 % aus einem konventionellen und einem modernen AD. Die stationäre Dauer bei moderner Monotherapie betrug 85 Tage, das Intervall bis zur Wiederaufnahme 182 Tage. Die Verweildauer bei konventioneller Monotherapie betrug 48 Tage, die freie Zeit bis Wiederaufnahme 200 Tage. Die stationäre Dauer bei Kombinationstherapie (neu-neu) betrug 65 Tage, die freie Zeit bis zur Wiederaufnahme 159 Tage. Die Verweildauer bei Kombinationstherapie (neu - konventionell) betrug 107 Tage, die freie Zeit bis zur Wiederaufnahme 209 Tage. Schlussfolgerung Grund für die hohe Zahl an Kombinationstherapien ist vermutlich die nicht ausreichende Wirksamkeit einer Monotherapie. Die längere Verweildauer bei Monotherapie könnte in der Einstellungsphase, die raschere Wiederaufnahme im ambulanten Setting begründet sein. Bei Kombinationstherapie ergeben sich sowohl für Verweildauer als auch das rückfallsfreie Intervall deutliche Vorteile für eine Kombination aus zwei modernen Substanzen.
Abstract
Objective In the treatment of depression conventional antidepressants have not entirely been replaced by their succeeding antidepressants which have considerably increased in numbers of prescription. We performed a systematic review analysis of a sample of in-patients with depressive disorder (n = 312) intended by the aim to define the influence of the applied antidepressant medication on the numbers of necessary readmissions. Methods We listed single antidepressant medication as well as combinations of conventional and new antidepressants as well as combinations with neuroleptics, tranquilizers and MAO-inhibitors. Results Average length of stay: 54 days, free days until readmission: 112 days. 26 % of the patients were treated with a single antidepressant medication of a new, 2 % of a conventional, 54 % with a combination of two new and 18 % with a combination of a conventional and a new antidepressant. Conclusions The main reason for treatment with antidepressant combinations might be that single medication did not suffice in efficacy according to the depressive symptoms. In further evaluations our special interest among others will be turned to assessing the significance of changes of medication on the outpatient basis on the clinical course.
