Differenzialdiagnose mediastinaler Raumforderungen

Zusammenfassung

In diesem Beitrag werden zunächst die zur Verfügung stehenden bildgebenden Untersuchungsverfahren dargestellt, die eine Beurteilung der Lage und Ausdehnung mediastinaler Raumforderungen ermöglichen. In der Folge werden Pseudotumoren, vaskuläre Veränderungen und sekundäre Raumforderungen vorgestellt, die von echten Tumoren differenziert werden müssen. Die sich daran anschließende Charakterisierung der einzelnen Raumforderungen ist im Wesentlichen nach topographischen Gesichtspunkten (vorderes, mittleres und hinteres Mediastinum) strukturiert.

Der Beitrag zeigt, dass sich die differenzialdiagnostische Einordnung an der Lokalisation und Morphologie der raumfordernden Läsionen, am Alter des Patienten und an der klinischen Symptomatik orientiert. Eine eindeutige Zuordnung der Läsion bleibt trotzdem häufig schwierig. Die wesentliche Aufgabe der Bildgebung ist es daher, auf der Basis der erhobenen Befunde das weitere diagnostische oder therapeutische Procedere zu lenken.

Abstract

This contribution first describes the different imaging modalities that are used for the detection and classification of mediastinal masses. Pseudotumors, vascular lesions and secondary masses are described, which have to be differentiated from primary tumors. The most common primary mediastinal lesions are presented, ordered according to their anatomical localization and imaging characteristics. It is lined out, how the differential diagnosis is influenced by the localization of the lesion, the age of the patient and the presence of clinical symptoms and how the imaging procedures can guide the choice of the invasive diagnostic approach.
