Bilateral traumatic castration, if left untreated, can result in permanent sexual, social, and psychological maladjustments. This was the report of a novel procedure of temporary ectopic replantation and refabrication of a traumatically avulsed testis of a 21- year-old unmarried male, with subsequent successful orthotopic microsurgical transfer to the perineum. This was necessary, as the length of the vessels was very limited and the viability of the testis was doubtful initially. The significance of this replantaiton was related to the refabrication of a long pedicle for subsequent reattachment to the perineum, which would not have been possible but for the ectopic replantation. Testosterone levels have remained within normal limits after the ectopic replant and orthotopic transfer.
