Wirkmechanismen von Probiotika

Zusammenfassung

Probiotika sind lebende Mikroorganismen, die den Darm aktiv und in ausreichender Menge erreichen und die Gesundheit des Wirtes positiv beeinflussen. Ihnen werden verschiedene gesundheitsfördernde Effekte zugeschrieben, doch das Wirkprinzip ist für die Mehrzahl der beschriebenen Effekte unverstanden. Das Verständnis der zugrunde liegenden Wirkmechanismen könnte die Glaubwürdigkeit des probiotischen Konzepts erhöhen. Dieser Aufsatz diskutiert bekannte und mögliche Wirkungsweisen von Probiotika. Die durch Probiotika vermittelte Verminderung von Symptomen der Laktoseintoleranz ist nachweislich auf die Zufuhr bakterieller β-Galaktosidase zurückzuführen. Die geringere Inzidenz von rotavirusbedingten Durchfällen ist möglicherweise auf die durch Probiotika induzierte Änderung der Glykosylierungsstruktur der Oberfläche von Epithelzellen zurückzuführen, die mit einer verminderten Adhäsion von Viren einhergeht. Die mikrobielle Besiedlung des Darms beeinflusst die spezifische und unspezifische Immunantwort des darmassoziierten Immunsystems. Einige Effekte von Probiotika lassen sich möglicherweise mit einer Modulation der intestinalen Mikrobiota bzw. mit der Beeinflussung von Immunfunktionen erklären. Die Fähigkeit von Probiotika, Bakteriozine zu bilden und die Sekretion von Cryptdinen durch Paneth-Zellen zu induzieren, könnte einen Beitrag zur Abwehr pathogener Bakterien leisten. Die Herabregulation inflammatorischer Zytokine durch Probiotika ist ein möglicher Mechanismus, der die beobachtete Remissionserhaltung bei entzündlichen Darmerkrankungen wie Colitis ulcerosa und Pouchitis erklärt. Toll-like-Rezeptoren sind wichtige Komponenten des angeborenen Immunsystems, die für die beobachteten immunmodulatorischen Effekte von Probiotika von zentraler Bedeutung sein könnten. Die Mehrzahl der vorgeschlagenen und diskutierten Wirkmechanismen von Probiotika ist jedoch unbewiesen. Daher sind weitere Studien zum besseren Verständnis der beschriebenen Effekte von Probiotika notwendig.

Abstract

Probiotics are live microbial food ingredients that when consumed in sufficient amount confer beneficial health effects on the host. Probiotics have been proposed to exert various health-promoting effects. The understanding of the mechanisms underlying these effects could help to improve the credibility of the probiotic concept. The improvement of lactose intolerance symptoms by probiotics can be explained by the intake of bacterial β-galactosidase. The lower incidence of rotavirus caused diarrhea brought about by probiotics is possibly due to changes in the glycosylation pattern of epithelial cell surface proteins in response to probiotics. The microbial colonisation of the intestine affects the specific and non-specific immune response of the gut-associated immune system. Some probiotic effects can be explained by the modulation of the intestinal microbiota or of the immune functions. The ability of probiotics to synthesise bacteriocins and to induce the secretion of cryptdins by Paneth cells, could account for the protection of probiotics against pathogenic bacteria. The down-regulation of inflammatory cytokines by probiotics could contribute to improved maintenance of remission in inflammatory bowel disease, in particular ulcerative colitis and chronic pouchitis. Toll-like-receptors, which are important components of the innate immune system, may be important mediators of the observed immunmodulatory effects of probiotics. Further studies are required to enhance the knowledge of the mechanisms underlying observed probiotic effects.
