Deziduose und Endometriose der Appendix als Ursache einer akuten abakteriellen Appendizitis

Zusammenfassung

Die akute bakterielle Appendizitis in der Schwangerschaft stellt eine bedrohliche und folgenschwere Situation dar, die diagnostischer Genauigkeit und therapeutischer Intervention bedarf. Eine korrekte Diagnosestellung kann durch abgeschwächte oder untypische Symptome verzögert werden. Differenzialdiagnostisch kann eine ektope Dezidua (Deziduose) der Appendix eine ähnliche klinische Symptomatik wie eine akute bakterielle Appendizitis aufweisen, woraus sich ein diagnostisches Dilemma ergibt. Wir berichten über eine 35-jährige Schwangere mit zunehmenden rechtsseitigen Unterbauchschmerzen, bei der aufgrund der Verdachtsdiagnose einer akuten Appendizitis in der 24. Schwangerschaftswoche eine notfallmäßige Appendektomie durchgeführt wurde. Die feingewebliche Untersuchung der Appendix wies eine lokale, abakterielle Entzündung, ausgedehnte ektope Dezidua und Residuen einer Endometriose nach. Die auf dem Boden der vorbestehenden Appendixendometriose entstandene Deziduose verursachte klinische Symptome, welche eine akute bakterielle Appendizitis in der Schwangerschaft vortäuschten.

Abstract

Acute appendicitis during pregnancy remains a serious medical condition, requiring diagnostic accuracy. However, the correct diagnosis during pregnancy may be delayed due to blunted symptoms. Furthermore, acute bacterial appendicitis may be mimicked by symptomatic ectopic decidua (deciduosis) of the appendix. A 35-year-old pregnant woman presented with progressive lower right abdominal pain. Based on clinical symptoms and laboratory findings the diagnosis of acute appendicitis was made and the patient underwent emergency appendectomy in the 24th week of gestation. Histological examination revealed the concomitant presence of localised inflammation, extended ectopic decidua and residuals of appendiceal endometriosis. Concomitant extended ectopic decidua and endometriosis of the appendix in combination with localised inflammation may lead to symptoms mimicking acute bacterial appendicitis.
