Abstract
Whereas at the turn of the century in Germany antipsychiatry left intact the medical concept of disease and comprehension of the role of the physician and medical institutions, this sector was initially questioned vehemently in the conception of "psychothérapie institutionelle" in France and later in the reform efforts in Britain and North-America. This disclosure of the questionability of an argumentation in terms of pure exact science promoted by the tendencies of the theory of science (Frankfurt School) in the 1950s and 1960s encouraged these reform efforts and questioned the objectivism which had been dominant up to that time. However, for the clinical domain it also led to a profound crisis of legitimation and thus to a crisis of the self-conception of the physician, which is linked fundamentally to diagnosis, indication and responsibly justified medical intervention. It was especially difficult to react appropriately to the challenges of antipsychiatry for those psychiatrists who had clearly recognized the deficiencies or the nonfeasibility of an approach based exclusively on exact science as a source of legitimation for medical action. These psychiatrists had gone along with the subjectivistic trend change with the criticism of traditional associated with this, asymmetric, dyadic doctor-patient relationship. In this context, they had rejected objectivizing diagnostics and the apersonal disease model ("therapeutic community") in the hope that this would bring about humanization. To the extent that it became evident that the challenges of antipsychiatry converged in the question as to how the relationship of physician, patient and disease is to be defined within a psychiatric institution, the absence of an integrative perspective comprising clinical theory and practice became evident: both the subjectivistic and the objectivistic approaches did not consider (in absolute terms) the requirement for legitimation of medical action. However, it is recapitulated that each of the subaspects is indispensable for determination of the relationship between the doctor, patient and disease. In the future, a guideline for the balancing act required is seen in an orientation to the basic anthropological preconditions entailed in the given clinical situation.
Zusammenfassung
Während die Antipsychiatrie der Jahrhundertwende in Deutschland den medizinischen Krankheitsbegriff und das ärztliche Rollen- und Institutionsverständnis noch unberührt ließ, wurde dieser Bereich zunächst in der Konzeption der ,,psychothérapie institutionelle" in Frankreich und später in den angloamerikanischen Reformbestrebungen nachhaltig in Frage gestellt. Die von den wissenschaftstheoretischen Strömungen (Frankfurter Schule) der 50er und 60er Jahre geförderte Offenlegung der Fragwürdigkeit eines rein naturwissenschaftlichen Argumentationsduktus bestärkte diese Reformbestrebungen und stellte den bislang tonangebenden Objektivismus in Frage. Sie führte aber für den Bereich der Klinik auch zu einer tiefgreifenden Legitimationskrise und damit einer Krise des ärztlichen Selbstverständnisses, das mit Diagnostik, Indikationsstellung und verantwortetem ärztlichen Eingriff fundamental verknüpft ist. Gerade für diejenigen Psychiater, die den Mangel bzw. die Unmöglichkeit einer ausschließlich naturwissenschaftlichen Betrachtungsweise als Legitimationsquelle ärztlichen Handelns klar erkannt und die subjektivistische Wende mit der damit verbundenen Kritik der traditionellen, asymmetrischen, dyadischen Arzt-Patient-Beziehung, der Ablehnung der objektivierenden Diagnostik und des apersonalen Krankheitsmodells (,,therapeutic community") in der Hoffnung auf eine dadurch zu bewirkende Humanisierung mitvollzogen hatten, war es besonders schwierig, auf die Herausforderungen der Antipsychiatrie angemessen zu reagieren. In dem Maße, in dem deutlich wurde, daß die Herausforderungen der Antipsychiatrie in der Frage konvergierten, wie das Verhältnis von Arzt, Patient und Krankheit innerhalb einer psychiatrischen Institution zu bestimmen sei, wurde das Fehlen einer integrativen, klinische Theorie und Praxis umgreifenden Perspektive deutlich: Sowohl die subjektivistischen wie die objektivistischen Lösungsansätze verfehlten - verabsolutiert - das Legitimationserfordernis ärztlicher Handlungszusammenhänge. Jeder dieser Teilaspekte ist jedoch, so wird resümiert, zur Bestimmung des Verhältnisses von Arzt, Patient und Krankheit unverzichtbar. Eine Leitlinie für den verlangten Balanceakt wird künftig in einer Orientierung auf die in der vorgegebenen klinischen Situation enthaltenen anthropologischen Grundvoraussetzungen gesehen.
