Zusammenfassung
Bei 198 Angehörigen des Institutes für Anaesthesiologie der Universität München wurden mittels Radioimmunoassay die serologischen Parameter HBsAg, HBeAg, Anti-HBs, Anti-HBe, Anti-HBc und Anti-HAV bestimmt. 119 Untersuchte gehörten dem Pflegepersonal und 79 dem ärztlichen Personal an. 167 Proben wurden zum Zweck der Laborkontrolle in einem Referenzlabor (Max-von-Pettenkofer-Institut München) nachuntersucht.
Mittels Fragebögen wurden anamnestische Daten zu Alter, früheren Erkrankungen, früheren Transfusionen oder Globulingaben, Auslandsreisen und Infektionsfall in der Umgebung, Erkrankungssymptomen, bisheriger Berufstätigkeit sowie zur Gefährdung am Arbeitsplatz erfragt.
Insgesamt hatten 50 Untersuchte (= 25 %) Anti-HBs, 42 Untersuchte (= 21 %) Anti-HBc, 17 Untersuchte (= 5,5 %) Anti-HBe und 63 Untersuchte (= 31,5 %) Anti-HAV in nachweisbarer Menge im Serum. Anti-HBe wurde nur in Kombination mit anderen Antikörpern gefunden. Der Nachweis von Anti-HBs war häufiger als von Anti-HBc. HBsAg oder HBeAg war in keiner Probe enthalten.
Ärzte wiesen häufiger Antikörper gegen Hepatitis B auf (Anti-HBs = 32,5 %, Anti-HBc = 28,7 %) als Angehörige des Pflegepersonals (Anti-HBs = 20,7 %, Anti-HBc = 15,8 %). Beim Pflegepersonal war in 22,7 % beim ärztlichen Personal in 40,6 % der Fälle einer der Antikörper nachweisbar. Die Antikörper-Häufigkeit, sowohl bezüglich Hepatitis A-Virus, als auch Hepatitis B-Virus nahm mit Alter und Berufsdauer zu. Die Häufigkeit von Hepatitis Antikörpern im Arbeitsbereich Anästhesie ist hoch. Beim Vergleich mit bereits publizierten Ergebnissen ergibt sich für Antikörper gegen das Hepatitis B-Virus eine etwa 2,5mal so große Häufigkeit wie bei der Durchschnittsbevölkerung. Antikörper gegen Hepatitis A-Virus waren hingegen gleich häufig. Ein Vergleich mit der Antikörperhäufigkeit gegen Hepatitis B-Virus mit dem Durchschnitt des Krankenhauspersonals zeigt eine etwa l,4fach erhöhte Häufigkeit im untersuchten Kollektiv. Die unterschiedliche Häufigkeit innerhalb der Berufsgruppen kann durch unterschiedliche Altersstruktur erklärt werden. Die Häufigkeitszunahme mit dem Lebensalter bzw. der Berufszeitdauer wird durch die steigende Wahrscheinlichkeit mit dem Virus in Kontakt zu kommen erklärt.
Das Risiko einer beruflich erworbenen Infektion mit Hepatitis B-Virus ist innerhalb des Arbeitsbereiches Klinik erhöht. Arbeitende im Bereich der Anästhesie haben ein zusätzlich erhöhtes Risiko. Wegen des offenbar schwer vermeidbaren Kontaktes mit dem Hepatitis B-Erreger dürfte die aktive Immunisierung die bestmögliche Schutzmaßnahme darstellen.
Summary
198 members of the Institute of Anaesthesiology at Munich University were tested using radioimmunoassays for HBsAg, HBeAg, Anti-HBs, Anti-HBc, Anti-HBe and Anti-HAV. Of those tested, 119 were on the nursing staff, and 79 were physicians. 167 samples were re-examined in a reference laboratory (Max-von-Pettenkofer-Institute Munich), for control purposes. Questionnaire information was collected on age, previous diseases, transfusions and globulin injections, trips abroad and close contact with the disease, symptoms of disease, previous jobs and last job and risk of infection at the place of work. In all it was found that 50 (25 %) of the subjects had detectable quantities of Anti-HBs in the serum. In 42 cases (21 %) Anti-HBc was detected, in 17 (5.5 %) Anti-HBe, and in 63 cases (31.5 %) Anti-HAV. Anti-HBc was found only in combination with other antibodies. Anti-HBs was evident more often than Anti-HBc. No sample contained either HBsAg or HBeAg.
More antibodies against hepatitis B were detected amongst the physicians (Anti-HBs = 32.5 %, Anti-HBc = 28.7 %) than amongst members of the nursing staff (Anti-HBs = 20.7 %, Anti-HBc = 15.8 %). Identifiable amounts of at least one antibody were detected in 40.6 % of the samples taken from physicians and in 22.7 % of the samples from the nursing staff. The incidence of antibodies against hepatitis A and hepatitis B viruses increased with age and working life. There is a high incidence of hepatitis antibodies amongst those working in anaesthesiology. A comparison with previously published studies indicates that hepatitis B antibodies occur on the average 2.5 times more frequently than in the general population. For the hepatitis A antibody, however, the average frequencies are the same. The frequency of hepatitis B antibodies in the investigated group was approximately 1.4 times higher than the average for all hospital workers. The differentials between the two professional groups can be explained in terms of age differences. The increasing incidence with age, and more particularly, the length of working life, is explained in terms of the higher risk of contact with the virus. There is a high risk of hepatitis B virus infection during work within the operational confines of the clinic itself. Anaesthesia workers are subject to even greater risk. Since contact with the hepatitis B virus can obviously hardly be avoided, an active immunisation programme should be considered as the best possible means of protection.
