Zusammenfassung
Zur Beurteilung der Leistungsfähigkeit der Sonographie bei der Darstellung des Ausmaßes von Darmwandveränderungen wurden die Befunde des trans-kutanen Ultraschalls bei 78 Patienten mit Kolontumoren (6 Polypen über 1 cm Größe, 68 Kolonkarzinome, 1 Pan-kreaskarzinom, 1 Gallenblasenkarzinom und 2 Divertiku-litistumoren) entsprechend oder vergleichbar der TNM-Klassifikation von 1987 klassifiziert und mit dem histolo-gischen Befund verglichen. 62 Ultraschall-Klassifizierungen waren korrekt. Sensitivität und Spezifität der U/Tl-bis U/T4-Stadien reichten von 0,64 bis 0,89 bzw. 0,7 bis 1,0. Am Beispiel der Kolontumoren unterschiedlicher Dignität läßt sich so zeigen, daß der Ultraschall Darmwandveränderungen nicht nur qualitativ, sondern auch in ihrem Ausmaß mit akzeptabler Genauigkeit darstellt.
Abstract
In order to reveal sonographic accuracy in imaging gut wall lesions transcutaneous ultrasonic find-ings in 78 colonic wall tumours (6 polyps sized more than 1 cm, 68 colonic carcinomas, 1 pancreatic carcinoma, 1 gallbladder carcinoma and 2 benign diverticulitis tumours) were staged according or similar to TNM classifi-cation from 1987 and compared to histologic findings. 62 ultrasonic stagings were correct, sensitivity and specifity of U/TI to U/T4 stagings ranged from 0.64 to 0.89 resp. 0.7 to 1.0. Thus, in the example of colonic tumours (of either dignity) ultrasound not only showes quality dif-ferences as gut wall lesions, but also images depth of invasion as a quantitative feature in gut wall lesions with acceptable accuracy.
