Sonographisch gesteuerte perkutane Feinnadelpunktion raumfordernder Prozesse des Abdomens und Retroperitoneums

Zusammenfassung

In den letzten vier Jahren führten wir bei 180 Patienten 240 ultraschallgesteuerte perkutane Feinnadelpunktionen raumfordemder Prozesse des Abdomens und des Retroperitoneums durch. 71mal wurden zystische Läsionen und 169mal solide Raumforderungen punktiert. Von den 169 Punktionen solider Tumoren entfielen 67 auf das Pankreas, 58 auf die Leber, 31 auf die Niere und 13 auf andere Organe des Abdomens.

In 27% der Pankreaspunktionen und in 18% der Nierenpunktionen konnte kein beurteilbares Material gewonnen werden.

In 88% wurde ein richtig-zytologisch positives Ergebnis erzielt. In 12% ergab sich ein falsch-negativer zytologischer Befund. In keinem Fall wurde ein falsch-positiver Befund erhoben.

Bei den zystischen Läsionen wurden 5 Pankreaszysten therapeutisch punktiert, wobei ein Patient 8mal punktiert wurde. In einem Fall trat die einzige Komplikation aller Feinnadelpunktionen auf, eine Pankreatitis, die unter konservativer Therapie ausheilte.

Viermal wurde eine therapeutische Nierenzystenpunktion vorgenommen. Dabei wurden bei der größten Zyste 860 ccm Harn abgesaugt.

Die ultraschallgesteuerte perkutane Feinnadelpunktion hat sich als leicht zu handhabende, gefahr- und schmerzlose Methode zur Diagnose unklarer abdomineller und retroperitonealer Raumforderungen erwiesen. Außerdem ermöglicht sie therapeutische Punktionen von Pankreas- und Nierenzysten.

Abstract

In the past four years percutaneous ultrasonically guided fine needle aspiration biopsies of space occupying lesions of the abdomen and retroperitoneum were performed on 180 patients. There were 71 aspirations of cystic and 169 aspirations of solid lesions. Amongst the latter pancreas was first with 67 followed by the liver with 58, the kidney with 31 and other organs of the abdomen with 13 aspirations. No diagnostic material was obtained in 27 percent of aspirations of the pancreas and in 18 percent of kidney aspirations. The cytological diagnoses were right positive in 88 percent and false negative in 12 percent. There was no false positive result.

5 pseudocysts of the pancreas were aspirated therapeutically, in one case 8 times subsequently. The only complication of all aspirations was a pancreatitis which healed under conservative treatment.

There were 4 therapeutic aspirations of kidney cysts. 860 ml urine was drained from the largest cyst. We conclude that percutaneous ultrasonically guided fine needle aspiration biopsy is an easy, safe and painless method of diagnosis of space occupying lesions of the abdomen and retroperitoneum. The same applies to therapeutic aspirations of cysts of the pancreas and kidney.
