Zusammenfassung
Um die Aussagekraft von hochauflösenden Ultraschallverfahren bei der Beurteilung von Halslymphomen bewerten zu können, wurde eine Vergleichsuntersuchung zwischen Palpation, Sonographie und intraoperativer Situation durchgeführt. Bei 42 Patienten wurden 210 Kriterien überprüft. Dabei zeigte sich, dass bei der Palpation eine Fehlerquote von 23,8%, bei der Sonographie von 4,8% und bei der Kombination beider Methoden von 1% vorlag.
Im Ultraschall sind Lymphome ab einer Größe von 5 mm zu erkennen, während sie bei der Palpation erst ab einer Größe von 15 mm zu tasten sind.
Die hochauflösende Sonographie stellt zur Abklärung von fraglichen Lymphomen am Hals eine wesentliche Bereicherung der Diagnostik dar. Durch die genaue Beschreibung ist das Auffinden der Lymphome intraoperativ erleichtert und die Abgrenzbarkeit gegenüber den großen Halsgefäßen präoperativ gut zu beurteilen. Die Sonographie des Halses bietet somit eine hohe Aussagekraft. Sie ist für den Patienten nicht belastend, nicht eingreifend, leicht verfügbar und zeitlich wenig aufwendig.
Summary
In order to determine the value of high resolution ultrasonic procedures in the diagnosis of lymphomas of the neck, a comparative study of palpation, sonography and the surgical findings was undertaken. 42 patients were studied and 210 criteria evaluated.
The error incidence was found to be 23.8% for palpation, 4.8% for the sonography and 1% for the combination of the two methods.
The suprasonic procedure can detect a lymphoma from 5 mm. on, whereas palpation is only possible upward of 15 mm. Therefore, high resolution sonography enables a more precise evaluation and diagnosis of lymphomas of the neck. It facilitates the locating of the lymphomas during surgery and also allows the differentiation to the large vessels of the neck preoperatively. Thus, sonography of the neck is of high informative value there is no physical inconvenience or stress for the patient, and it is easy and quick to administer.
