Zur Problematik von Kombinationsbrüchen der Oto- und Rhinobasis

Zusammenfassung

Die Ursachen zur Entstehung von Kombinationsbrüchen der Otobasis und der Keilbeinhöhlenwandung werden diskutiert. Die Befunde bei vier Patienten mit solchen Frakturen werden beschrieben. Die isolierte Keilbeinhöhlenwandfraktur bei einer gleichzeitigen otobasalen Fraktur ist eine besonders gefährliche Kombination, weil sie schwer zu erkennen ist. Die operative Revision der Keilbeinhöhle ist vordringlich, um rhinogene Komplikationen zu vermeiden, obwohl möglicherweise die Symptome von Seiten der otobasalen Fraktur im Vordergrund stehen.

Summary

The causes of simultaneous fractures of the otobasis and the sphenoidal sinus are discussed. The findings in four patients with such combined fractures are described. The isolated fracture of the sphenoidal sinus combined with an otobasal fracture is most dangerous because diagnosis is difficult (cerebrospinal rhinorrhoea, signs originating from neighbouring structures; x-ray film findings do not always reveal a simultaneous fracture of the sphenoidal sinus. Surgery of the sphenoidal sinus is indicated mainly to prevent rhinogenic complications, although the signs of the otobasal fracture may be more prominent.
