Anmerkungen zur Verwendung des myokutanen Lappens des Musculus pectoralis major*

Zusammenfassung

Der myokutane Lappen des Musculus pectoralis major wird vielseitig verwendet. Seine Eignung zur Deckung von Haut- und Zungendefekten ist belegt. Sein Einsatz bei der Pharynxrekonstruktion ist gelegentlich mit Komplikationen wie Fistelbildungen belastet. Entstellungen der Brustregion, Wulstbildungen über der Klavikula u.a. regen zur Verfeinerung der Technik an. Die Wahl der Lappenentnahmeregion, die Schnittführung, die Präparation des Lappens, die Rekonstruktion des Entnahmedefektes und die Deckung des Operationsdefektes werden diskutiert. Die bisherigen Ergebnisse können verbessert werden.

Summary

The pectoralis major musculocutaneous flap represents a significant advancement in reconstructive surgery. It facilitates the immediate reconstruction of a variety of defects in the head and neck area especially in skin and tongue defects. Occasionally, after its application in pharynx reconstruction pharyngocutaneous fistulae occurred. Distorsion of the nipple, disfiguration of the chest, musclepedicle bulging on the clavicula, however, prompt surgeons to find an improvement of this technique. Design and position of the skin island, size and location of the skin incision, preparation of the pedicle, reconstruction of the skin island defect and grafting of the surgical defect are discussed. Previous results can be improved.
