Der prothetische Tracheaersatz mit Kunststoff*

Zusammenfassung

Sehr ausgedehnte bzw. in das Mediastinum reichende Trachealstenosen sind therapeutisch der Teamarbeit von Hals-Nasen-Ohrenarzt und Thoraxchirurg vorbehalten. Diese Zusammenarbeit sicherte durch Querresektion und End-zu-End-Anastomose in bisher 14 Fällen funktionell gute Resultate. Der plastischen Versorgung von Trachealstenosen sind durch Ätiologie und Größe der beteiligten Tracheal-Bronchialareale Grenzen gesetzt. Neville hat 1976 in 26 Fällen die Möglichkeit des prothetischen Ersatzes mit einem Silikon-Kautschuk-Polymer erprobt. Bei strenger Indikation mit notfallmäßig drohender Erstickung haben wir bei 4 Patienten einen Trachealersatz durch eine Neville-Prothese vorgenommen, über Indikation, Ergebnisse und Problematik wird berichtet.

Summary

Long tracheostenosis especially when sharing the mediastinum should be treated by a team of ENT- and thoracic surgeons. In 14 cases we achieved good functional results by transverse resection and anastomosis of the trachea. Plastic surgery of the trachea is limited by the etiology and the length of the shared tracheobronchial area. Neville (1976) has experiences in prosthetic replacement by synthetics in 26 cases. In 4 cases of special and cautiously placed indication we used Neville-prosthesis. The indications, the problems and the results are reported.
