The Concept of “Direct Mechanical Ventricular Assistance” in the Treatment of Left-Ventricular Failure - Part 2: Results of the New Intrathoracic Implantable Multi-Chamber Pump System (IMPS) in an Animal Study

An assist device was developed which is able to support the pumping function of the heart by direct application of pressure to the left ventricle. The goal of this animal study in pigs was to detormine whether it is possible to maintain sufficient blood circulation with the aid of the new system when the heart is fibrillating or its capacity has been greatly reduced. Following sternotomy complete invasive monitoring was installed. The intrathoracic implantable mechanical multi-chamber pump system (IMPS) was placed around the left ventricle. By means of the beta-blocker Carazolol, systolic left-vcntricular pressure (LVP_sys), cardiac Output, heart rate, and left-ventricular dp/dt_max (LVdp/dt_max) were gradually lowered and the pump System was tested intermittently. Then the heart was fibrillated and the System was tested again. When cardiac Output, LVdp/ dt_max, and systolic blood pressure were reduced by approximately 50 %, IMPS was able to increase LVP_sys by 83 % (IMPS on: 96± 9 mmHg vs. IMPS off: 63 ±6 mmHg), and the blood pressure in the carotid artery by 86% (IMPS on: 95/ 40 ± 15 mmHg vs. IMPS off: 69/38 ± 9 mmHg). The mean cardiac output was 64 % (IMPS on: 4.3 L/min vs. IMPS off: 3.9 L/min); in most cases a great variability could be observed depending on the preload, the heart rate, and the mode of pressure application. When the heart was fibrillating, IMPS was able to maintain adequate circulatory conditions with LVP_sys = 88 %, blood pressure in the carotid artery = 85 %, and LVdp/dt_max = 57% of tho control values measured before fibrillation and β-blockade.

The system presented here is able to support the impaired left ventricle and to replace its pumping funetion. The advantages of tho system are its efficiency and the lack of contact of the circulating blood with foreign surfaces. Whether the system is suited for bridging and recovery support shall be clarified in further studies.

Es wurde ein intrathorakal implantierbares mechanisches Mehrkammer Pumpsystem (IMPS) entwickelt, das durch direkte Druckanwendung am linken Ventrikel dio Pumpfunktion des Herzens unterstützen oder übernehmen soll. Ziel der vorliegenden tierexperimentellen Studie war es zu klären, ob es mit Hilfe des neuartigen Systems möglich ist, am β-blockierten und am flimmernden Herzen suffiziente Kreislaufverhältnisse aufrechtzuerhalten bzw. zu erzeugen. 8 Schweine mit einem Gewicht von 65-80 kg wurden median sternotomiert und einem kompletten invasiven Monitoring zugeführt. Danach erfolgte die Plazierung des Mehrkammerpumpsystems um den linken Ventrikel. Mit Hilfe des β-Blockcrs Carazolol wurde dann stufenweise der systolische linksventrikuläre Druck (LVP_sys), das Herzminutenvolumen, die Herzfrequenz und die linksventrikuläre Kontraktionsgeschwindigkeit (LVdp/dt_max) gesenkt und das Pumpsystem intermittierend getestet. Danach wurde das Herz zum Flimmern gebracht und das System erneut getestet. Es zeigte sich, daß nach einer ungefähr 50 %igen β-Blockierung das System in der Lage war, einen LVP_sys von 83 % des Kontrollwertes zu erzeugen (IMPS an: 96 ±9 mmHg vs. IMPS aus: 63 + 6 mmHg) und einen Blutdruck in der A. carotis von 86% (IMPS an: 95/40 ±15 mmHg vs. IMPS aus: 69/38 ±9 mmHg). Das Herzminutenvolumen wurde auf 64% erhöht (IMPS an: 4,3 l/min vs IMPS aus 3,9 l/min), es zeigten sich im Einzelfall deutliche Schwankungen, bedingt durch dio Vorfüllung des Herzens, die Frequenz der Druckanwendung und deren Ort. Am flimmernden Herz erreichte das System Werte von 88 % für den LVP_sys, 85 % für den Blutdruck in der A. carotis und 57 % für LVdp/dt_max in Prozent vom Ausgangswert.

Das hier vorgestellte Unterstützungssystem ist in der Lage, den Druck im linken Ventrikel, den peripheren Blutdruck, LVdp/dt_max und das Herzminutenvolumen zu erhöhen. Es ist möglich, den kontraktionseingeschränkten oder versagenden linken Ventrikel zu unterstützen bzw. dessen Pumpfunktion komplett zu übernehmen. Als Vorteile des Systems sind in erster Linie seine Effizienz und der fehlende Kontakt dos zirkulierenden Blutes mit Fremdoberflächen zu nennen. Weitere Untersuchungen müssen klären, ob das System auch für längerdauernde Unterstützungen geeignet ist.
