Summary
To regain blood shed intraoperatively, two different systems are clinically established: washing and centrifuging red blood cells to produce autologous erythrocyte concentrates and devices for immediate reinfusion of whole blood after mere filtration. In a prospective-randomised study to compare both methods regarding their efficiency, adverse effects, and economy, 20 patients of our department undergoing elective aortoiliac surgery received intraoperative autotransfusion by means of either cell washing (CS) or salvage of whole blood (WB). Patients were preoperativoly randomized into one of the two groups and were evaluated with respect to standard metabolic and haematological laboratory parameters preoperatively, during surgery, after transfer into the recovery room, 24 h after surgery, and at discharge. Both patient groups were well comparable in demographics, preoperative laboratory data, and indication for operation. Handling was easier, the set-up time was shorter with the whole blood filtration device (10.2±2.3 versus 21 ±1.9min, p = 0.0023), and no additional personnel was needed to run the system. The whole blood device also allowed a greater percentage of aspirated blood to be returned intraoperatively compared to cell washing (73.5%±7.2 versus 51.1 %±6.5, p = 0.03). Thrombocytopenia occurred in 7 (CS) and 3 (WB) patients intraoperatively with a significant difference in platelet count between the two groups (118 ± 17 [CS] versus 170 ± 12 [WB] * 109/L, p = 0.025). Expected changes in the perioperative course of the clotting parameters such as highly increased PTT levels and moderately prolonged Prothrombin times occurred in all cases. These parameters returned to their preoperative levels already in the early postoperative period, causing no clinical manifestation of bleeding tendency. Plasma-free haemoglobin was higher in the WB group intraoperatively (52.2 ± 15.4 versus 8.7 ±2.1 mg/dl, p = 0.02) and in the recovery room (42.4 ± 13.2 versus 6.6 ± 1.5 mg/dl, p = 0.025) returning to normal already on the first postoperative day. Clinical or laboratory-defined hints for renal failure and symptomatic adverse effects of autotransfusion did not arise in either group, nor did any other clinical complications arise in the intraoperative and early postoperative period.
In our study autotransfusion of whole blood proved to be more efficient and economical. The temporary increase of plasma-free haemoglobin is a well-known, intrinsic effect of whole blood reinfusion and showed no adverse effects in our patients. We therefore recommend the further utilization of whole blood autotransfusion devices as alternative to cellsaver systems in aortoiliac vascular surgery.
Zur Aufbereitung des während einer Operation steril abgesaugten Blutes für eine Autotransfusion sind zwei unterschiedlich arbeitende Systeme klinisch etabliert: Systeme zur Herstellung von Erythrozytenkonzentraten durch Waschen und Zentrifugieren des Aspirats sowie Apparate zur Retransfusion von filtriertem Vollblut. Im Rahmen einer prospektiv randomisierten Studie wurden bei 20 Patienten unserer Klinik die beiden Autotransfusionsprinzipien “Zell-Waschen” (CS) und “Vollblutretransfusion” (WB) bei elektiven Gefäßrekonstruktionen im Bereich der abdominellen Aorta im Hinblick auf Effektivität, Nebenwirkungen und auch Wirtschaftlichkeit verglichen. Nach Randomisierung jedes Patienten wurden laborchemische und hämatologische Parameter prä- und intraoperativ, nach Transfer auf die Aufwachstation, 24 Std. postoperativ und bei der Entlassung erhoben und gegenübergestellt. Die Patientengruppen waren in bezug auf demographische Daten, präoperative Laborwerte und Art der erforderlichen Operation vergleichbar. Vorteile der Vollblutfiltration bestanden hinsichtlich der einfacheren Handhabung, der set-up-Zeit des Systems (10,2 ±2,3 versus 21 ±1,9 min, p = 0.0023) und des Verhältnisses retransfundiertes Volumen/Blutverlust (73, ±5 7,2% versus 51,1 ± 6,5%, p = 0.030). Bei sieben (CS-Gruppe) bzw. drei Patienten (WB-Gruppe) trat eine intra- bzw. postoperative Thrombozytopenie auf, die Thrombozytenzahl war im Vergleich beider Gruppen deutlich unterschiedlich (118 ± 17 [CS] versus 170 ± 12 [WB], *109/ L, p = 0.0256). Erwartete Veränderungen im Verlaufweiterer Gerinnungsparameter wie ein starker Anstieg der partiellen Thromboplastinzeit sowie eine mäßige Verlängerung der Prothrombinzeit wurden in beiden Gruppen beobachtet. Diese Parameter normalisierten sich frühpostoperativ, und es wurden auch keine gerinnungsbezogenen Komplikationen beobachtet. Das freie Hämoglobin im Serum war in der WB-Gruppe gegenüber der CS-Gruppe sowohl intra- (52,2 ± 15,4 versus 8,7 ± 2,1 mg/dl, p = 0,02) als auch früh postoperativ (42,4 ± 13,2 versus 6,6 , ± 5 mg/dl, p = 0.025) stark erhöht, normalisierte sich aber wieder ab dem ersten postoperativen Tag. Des weiteren blieben laborchemischen Parameter in beiden Gruppen im Normbereich. Nierenfunktionsstörungen und auch sonstige klinisch erfaßbare Nebenwirkungen durch die Autotransfusion wurden bei keiner Gruppe beobachtet.
In unserer Serie hat die Autotransfusion von filtriertem Vollblut hinsichtlich Effektivität und Wirtschaftlichkeit besser abgeschnitten. Der systemimmanente temporäre Anstieg des freien Hb in dieser Gruppe zeigte in unserem Krankengut keine negativen Begleiteffekte für den Patienten. Die Vollblutautotransfusion kann daher in der aortoiliakalen Gefäßchirurgie als Alternative zum Cell-Saver empfohlen werden.
