Summary
We report the case of a 52-year-old male who underwent total orthotopic heart transplantation for end-stage ischemic car diomyopathy. The postoperative course was complicated by acute intestinal infarction which was diagnosed after surgical exploration, and treated with a subtotal colectomy with Brooke ileostomy and closure of the distal sigmoid three days post transplant. The patient survived with nutritional support and broad antibiotic Prophylaxis. Review of the literature on acute abdominal complications after operations involving cardiopul monary bypass suggests that such complications are usually fatal. Detection and diagnosis may be obscured and treatment complicated by immunosuppression after cardiac transplanta tion. Because of the poor prognosis without appropriate man agement, a high level of suspicion, early and aggressive diag nostic measures, and swift surgical intervention are essential to survival.
Zusammenfassung
Wir berichten über einen 52jährigen Mann, der wegen isch ämischer Cardiomyopathie eine totale orthotope Herztrans plantation erhielt. Im postoperativen Verlauf kam es zu einem akuten Mesenterialinfarkt, der im Rahmen einer explorativen Laparotomie diagnostiziert und mit einer subtotalen Kolekto mie mit Brooke-Ileostomie und Verschluß des distalen Sigmas am dritten Tag nach Transplantation versorgt wurde. Der Pa tient überlebte mit parenteraler Ernährung und breiter anti biotischer Abdeckung. Eine Untersuchung der Literatur über akute abdominelle Komplikationen nach Operationen mit Hilfe des kardiopulmonalen Bypass zeigt, daß solche Komplikatio nen meistens letal sind. Diagnose und Behandlung werden er schwert durch die Immunsuppression nach Organtransplanta tion. Aufgrund der schlechten Prognose ohne adäquates Ma nagement sind ein hohes Maß an klinischem Verdacht, die frühe und aggressive Diagnostik und ein rasches chirurgisches Vorgehen entscheidend für das Überleben des Patienten.
