Summary
A case of malignant lymphoma in the thoracic paravertebral region with a severe spinal cord compression syndrome is presented. The patient was a 59-year-old female who had had right hypochondralgia as initial symptoms for more than one year. Chest radiograph and computed tomographic (CT) scanning on admission to another hospital had demonstrated a mass lesion in the thoracic paravertebral area. Due to the subsequent occurrence of progressive cord compression syndrome, she was transferred to our institution for the purpose of surgical treatment. Myelography demonstrated an obstruction of the spinal canal at the level of the 8th thoracic vertebra. CT-myelography showed a paravertebral tumour which extended through the intervertebral foramen into the epidural space and also spread to the posterior thoracic wall in succession. Tumour extirpation and decompressive laminectomy by single stage operation were peformed. The pathological diagnosis was extranodal malignant lymphoma, diffuse, medium-sized cell type, with immunohistological demonstration of B-cell markers. Postoperatively, she was given a combination chemotherapy with VEPA (vincristine, cyclophosphamide, Prednisone and doxorubisin), and the functional disorder has improved to some extent. To our knowledge, this is the first case of soft tissue malignant lymphoma in the posterior mediastinum.
Zusammenfassung
Es wird über eine 59jährige Frau berichtet, die ca. 1 Jahr lang über Rückenschmerzen und eine rechtsseitige Hypochondralgie klagte. Mit Hilfe eines Thorax-Röntgenbildes und eines thorakalen CT konnte ein paravertebraler Tumor rechts festgestellt werden. Aufgrund eines progressiven spinalen Kompressionssyndroms mit Parästhesien und motorischer Schwäche der unteren Extremitäten wurde sie dann stationär aufgenommen.
Eine Myelographie ließ eine Obstruktion des Spinalkanals auf Höhe des 8. Brustwirbels erkennen, die CT-Myelographie zeigte einen paravertebralen Tumor, der wie eine Sanduhrgeschwulst durch das Foramen intervertebrale in den Epiduralraum vorgewachsen war. Intraoperativ wurde sichtbar, daß der Tumor die Dura mater noch nicht infiltriert hatte, er konnte en bloc entfernt werden, eine Dekompression mittels Laminektomie erfolgte in gleicher Sitzung. Bei der pathologischen Aufarbeitung des entnommenen Materials wurde ein extranodulär, diffus wachsendes malignes Lymphom vom mittelgroßen Zelltyp diagnostiziert, B-Zell-Marker waren vorhanden. Postoperativ wurde eine Chemotherapie mit Vincristin, Cyclophosphamid, Prednison und Doxorubisin ausgeführt. Heute, 19 Monate nach dem Eingriff, ist die Patientin rezidivfrei, die gesamten Umfelduntersuchungen sind negativ. Unseres Wissens nach ist dies der erste berichtete Fall eines malignen Lymphoms im hinteren Mediastinum.
