Summary
Multiple primary lung cancers are now being recognized more frequently. Out of 1004 patients with a resected lung cancer, 32 (3.2 %) presented a second primary and two of them a third primary lung cancer. A synchronous (S) primary was present in 7 and a metachronous (M) primary was present in 25. The histology was different in 1/7 with a S. and in 6/25 with a M. cancer. In all cases the cancer was located in another segment, lobe or lung. Out of the 7 with a S. cancer, 3 had a one stage and 4 a staged resection; out of the 25 with a M. cancer, 3 were treated by chemotherapy, 22 had a 2nd and 1 a 3rd operation to remove a new cancer. The mean time interval in M. cancer was 4 years, 8 months and was longer for adeno- (8 years) than for squamous cell carcinomas (4 years), longer for a contralateral cancer (6 years, 7 months) and longer for mild smokers (5 years, 1 month). The early mortality (10.3 %) was 0/7 for S. and 3/22 for M. cancers. The three and five years actuarial survival of operated patients was 67 % and 25 % for S. and 43 % and 31 % for M. cancers. Survival is positively affected by a resection interval of more than 3 years and by 3 instead of 2 remaining lobes after the second resection.
In conclusion a close follow-up of operated lung cancer patients is necessary and aggressive surgical approach is indicated for a new primary cancer.
Zusammenfassung
In einer retrospektiven Studie wird über 1004 Patienten berichtet, die wegen eines Bronchialkarzinoms operiert wurden. Bei 32 (3,2 %) von ihnen wurde ein zweites und bei 2 (0,2 %) ein drittes primäres Bronchialkarzinom diagnostiziert.
Das Zweitkarzinom wurde bei 7 Patienten gleichzeitig (Gruppe S = synchronous) mit dem ersten festgestellt - dabei zeigte sich in einem Fall eine unterschiedliche Histologie -, bei 25 Patienten trat es nach dem ersten Karzinom auf (Gruppe M = metachronous), hier wurde 6mal eine andere Histologie gesehen. Immer war der Tumor in einem anderen Segment, Lappen oder Flügel lokalisiert. Von den 7 Patienten der Gruppe S wurden 3 in einer Sitzung und 4 mehrmals operiert. In Gruppe M wurde 3mal eine Chemotherapie, 22mal eine Zweit- und einmal ein Dritteingriff ausgeführt.
Das mittlere Zeitintervall zwischen Erst- und Zweitkarzinom betrug 56 Monate, es verlängerte sich auf 96 Monate, wenn es sich um ein Adenokarzinom handelte, auf 79 Monate, wenn ein Tumor auf der kontralateralen Seite auftrat, und auf 61 Monate, wenn der Patient ein nur schwacher Raucher war. Die Frühsterblichkeit (10,3 %) lag in Gruppe M bei 13,6 %, in Gruppe S verstarb kein Patient. Die 3- bzw. 5-Jahres-Überlebensrate betrug in Gruppe S 67 % bzw. 25 %, in Gruppe M 43 % bzw. 31 %. Positiv beeinflußt wurde das Überleben, wenn zwischen erster und zweiter Resektion ein Intervall von mehr als 3 Jahren lag, und wenn 3 anstatt 2 Lappen erhalten blieben.
Nach Meinung der Autoren zeigen diese Ergebnisse, daß lückenlose Nachkontrollen bei Patienten mit operierten Bronchialkarzinomen notwendig sind und ein aggressives chirurgisches Vorgehen bei wiederholtem Auftreten eines primären Karzinoms indiziert ist.
