Summary
Severe ostial stenosis of the coronary arteries following aortic valve replacement is a potentially lethal complication. The usual presentations are recent onset of severe angina, ventricular arrhythmias, congestive heart failure, and sudden death. It is generally accepted to arise from injury to the coronary arteries during direct cannulation and continuous perfusion of cardioplegia under high pressure during operation. We report on a patient who developed critical left coronary ostial stenosis after aortic valve replacement. The cause for the stenosis was probably related lo the over-sizing and orientation of the prosthesis. The prosthesis was replaced and patch angioplasty of the left coronary ostia performed. The patient was well with normal coronary anatomy three years after surgery.
Zusammenfassung
Eine schwere Ostiumsienose der Koronararterien nach einem Aorlenklappenersatz ist eine mögliche tödliche Komplikation. Die üblichen Manifeslationen sind eine plölzlich auflrelende schwere Angina, ventrikuläre Arrhythmien. eine Herzinsuffizienz und plötzlicher Tod. Es ist allgeniein anerkannt, daß es durch die direkte Kanülierung und kontinuierliche Perfusion mil Kardioplegieunter hohem Druck wührend der Operation zu cincr Schädigung dcr Koronarien kommen kann. Wir berichlen über eine Arterie, die nach einem Aerlenklappenersatz. eine kritische Stenose des linken Koronararlcricnostiums enlwikkelte. Der Grund für diese Slcnose stand möglicherwcise in Zusammenhang mil einer iu großen und schlccht positionierten Prothese. Die Prothese wurde durch eine kleinere ersetzt und das linke Koronarostium durch eine Palehplastik envoitert. Drei Jahre nach der Operation ist die Patientin wohlauf, sie zeigt eine normale Koronaranalomie.
