Abstract
The present paper forms the second part of a critical evaluation of the use of pharmaco-EEG as an instrument for assessing the profile of action of psychoactive drugs in man based on the trial results of three new psychoactive test substances. Part I described the basic principles and methodological approaches and illustrated the value of pharmaco-EEG in framing hypotheses about the therapeutic efficacy of psychotropic drugs by the example of the neuroleptic drug savoxepine. This chapter illustrates the relevance of pharmaco-EEG in the assessment of psychoactive drugs by reference to the test substance levoprotiline. Levoprotiline is the pure R-(-)-enantiomer of the racemic drug oxaprotiline, a successor to the second-generation antidepressant maprotiline. Only the S-(+)-enantiomer of oxaprotiline inhibits the re-uptake of noradrenaline. In view of the absence of any monoamine-uptake inhibiting action of levoprotiline, this drug was assumed to be devoid of an antidepressive action. This assumption, however, was disproved by observations made in clinical studies with depressive patients, in whom levoprotiline did indeed desplay antidepressive activity. Investigations of levoprotiline in the pharmaco-EEG model - even though being retrospective in the sequence of events - would have been able to produce objective prediction of antidepressive potential in man. The substance was tested by comparison to placebo and to the standard antidepressant imipramine in nine young, healthy male volunteers. The experimental design was a cross-over uncompleted block design with three consecutive trial days one week apart. The pharmaco-EEG was recorded from the occipital region before 2h and 4h after administration of single doses of 150mg levoprotiline, 75 mg imipramine and placebo. The selected target variables were the Spectral Difference Index (SDI), the relative and absolute power values in seven predetermined frequency bands within the range 1.5 to 30.0 Hz and the dominant frequency (6.0 to 18.0 Hz). - Analysis of the relative and absolute power values in the seven predetermined frequency bands showed that levoprotiline has a profile of action similar to that of imipramine, i. e. a decrease of relative and absolute powers in the alpha range and an increase in the slow-wave range, as well as an increase of the relative power and decrease of the absolute power in the beta range. - The results indicate that pharmaco-EEG can make an important contribution to the assessment of the profile and quality of action of a potential new antidepressant. - Levoprotiline had like imipramine effects on vigilance regulation after acute administration in healthy subjects, - From a psychophysiological point of view, depressive disorder may be a disease of the serotonergic and thus the vigilance regulating system. Thus - according to our theory - in this human pharmacological model partial effects of therapeutic efficacy may be reflected.
Zusammenfassung
Der vorliegende Beitrag bildet den zweiten Teil einer kritischen Bewertung des Pharmako-EEG als Meßinstrument zur prospektiven Bewertung des Wirkprofils von Psychopharmaka auf der Basis der Prüfungsergebnisse von drei neu entwickelten psychoaktiven Prüfsubstanzen. Teil I behandelte zunächst die allgemeinen Prinzipien und methodischen Ansätze und erläuterte die Relevanz des Pharmako-EEG bei der Hypothesenbildung über die therapeutische Wirksamkeit von Psychopharmaka am Beispiel der neuroleptisch wirksamen Prüfsubstanz Savoxepin. - Bei der hier betrachteten Prüfsubstanz Levoprotilin handelt es sich um das reine R-(-)-Enantiomer des Razemats Oxaprotilin, einem Nachfolger des der zweiten Generation angehörenden Antidepressivums Maprotilin. Auf der Auftrennung von Oxaprotilin in seine beiden Enantiomere konnte in pharmakologischen Untersuchungen gezeigt werden, daß eine Noradrenalin-Wiederaufnahmehemmung, die nach der Monoamin-Hypothese als neurobiochemischer Wirkmechanismus der antidepressiven Wirksamkeit gilt, ausschließlich durch das S-(+)-Enantiomer erfolgt. Der gänzlich fehlende Einfluß des R-(-)-Enantiomers Levoprotilin auf das monoaminerge System ließ dementsprechend auf das Fehlen einer antidepressiven Wirksamkeit schließen. Klinische Studien hingegen ergaben, daß Levoprotolin durchaus antidepressiv wirksam ist. - Die Prüfung von Levoprotilin im Pharmako-EEG-Modell erbrachte ebenfalls Evidenzen für ein antidepressives Wirkpotential. Geprüft wurde die Test- Substanz im Vergleich zu Placebo und dem Standardantidepressivum Imipramin an neun jungen, gesunden, männlichen Probanden. Die Gabe der Prüfsubstanzen erfolgte an drei im Abstand von je einer Woche aufeinanderfolgenden Prüftagen. - Das Pharmako-EEG wurde zu den Zeitpunkten prä sowie 2 h und 4 h post applicationem von jewils 150 mg Levoprotilin, 75 mg Imipramin bzw. Plazebo von der Okzipitalregion abgeleitet. Die Zielvariablen waren Spectral Difference Index (SDI), relative und absolute Powerwerte in sieben vorgegebenen Frequenzbändern innerhalb des Frequenzbereiches 1,5 bis 30,0 Hz sowie die dominante Frequenz (6,0-18,0 Hz). - Aus den Ergebnissen läßt sich ableiten, daß Levoprotilin EEG-Effekte hervorruft, die denen von Imipramin ähnlich sind: Abnahme der relativen und absoluten Power im Alpha-Bereich, Zunahme der relativen und absoluten Power im Bereich langsamer Wellen sowie Zunahme der relativen Power und Abnahme der absoluten Power im Beta-Bereich. - Darüber hinaus ergab sich bei der Klassifizierung von Levoprotilin mit Hilfe des objektiven Klassifikationssystems für Psychopharmaka die höchste Zuordnungswahrscheinlichkeit zu der Klasse der Antidepressiva. - Diese Ergebnisse sprechen für die Validität des Pharmako-EEG-Modells zur prospektiven Bewertung des Wirkprofils von antidepressiv wirksamen Prüfsubstanzen. - In der Vorstellung der Psychophysiologie ist die depressive Erkrankung eine Störung sowohl der affektiven als auch der Vigilanzregulation. Die Darstellung von Einflüssen auf die Vigilanzregulation gesunder Probanden nach einmaliger Applikation bildet unserer Ansicht nach Partialeffekte der depressiven Erkrankung in einem klinisch-pharmakologischen Modell ab.
