Summary
In order to assess the value of dobutamine stress echocardiography (DSF) for detecting coronary artery disease (CAD), 67 consecutive patients (mean age ± SD was 58 ±8 years, range 35 to 75; 46 men and 21 women) with known or suspected CAD undergoing selective coronary angiography within the week following the DSE were studied. Two patients were excluded from the study because of insufficient echocardiographic imaging. Dobutamine (5 to 30 μg/kg/min by 5 μg/kg/min increments) was infused in 5-minute intervals. All the patients had 12-lead electrocardiogram (ECG) recorded at rest and at each stage of dobutamine infusion.
There was significant CAD (≥ 50 % diameter Stenosis) in 42 patients (64.6%), with 16 patients having 1-vessel, 7 patients having 2-vossel and 19 patients having 3-vossol CAD. In 22 patients coronary angiogram was normal. DSE was positive in 33 of 42 patients with CAD. The test was negative in 20 of 23 patients without CAD. Compared with coronary angiography, the overall sensitivity of DSE for detecting CAD was 78.6%, specificity 87%, positive predictive value 91.7%, negative predictive value 69 %, and accuracy 81.5 %. The sensitivity in those with one-vessel, two-vessel, three-vessel and multivessel disease was 62.5 %, 85.7 %, 94.7 %, and 92.3 % respectively. DSE was well tolerated in all patients. The study was not prematurely terminated due to a side effect in any patient.
This study indicated that DSE is a practical method for the non-invasive assessment of significant CAD, for determining the patients who require invasive tests, and for predicting the extent of disease. It is concluded that DSE is a safe, versatile, totally non-invasive, and clinically useful test.
Zusammenfassung
In einer Untersuchung an 67 Patienten (46 Männer und 21 Frauen, Altersdurchschnitt 58 ± 8 Jahre) mit bekannter oder vermuteter koronarer Herzkrankheit (KHK) wurde der diagnostische Wert der Dobutamin-Belastungs-Echokardiographie (DSE) bestimmt, indem deren Ergebnis eine Woche später mit einer selektiven koronaren Angiographie verglichen wurde. Dobutamin wurde in einer Dosierung von 5-30 μg/kg/min in 5minütigen Intervallen bei jeweiliger Dosissteigerung von 5 μg infundiert. Das EKG wurde mit 12 Ableitungen in Ruhe und bei jeder Dobutamin-Dosierungsstufe registriert. Eine beträchtliche Koronarstenose (50 % Einengung und darüber) fand sich bei 42 Patienten, wobei 16 Patienten eine 1-Gefäßkrankheit, 7 Patienten eine 2-Gefäßkrankheit und 19 Patienten eine 3-Gefäßkrankheit hatten. In 22 Fällen war das Koronarangiogramm normal. Die DSF war in 33 Fällen von 42 Patienten mit KHK positiv. Bei den 23 Patienten ohne KHK war in 20 Fällen die DSE negativ. Die Sensivität der DSE bei Entdeckung einer KHK im Vergleich zur Koronarangiographie betrug 78,6%, die Spezifität 87 %. In Bezug auf die Anzahl der betroffenen Gefäße betrug die Sensivität bei 1-, 2-, 3 und Mehrgefäßkrankheit 65,5%, 85,7%, 94,7% und 92,3%. Die DSE wurde von den Patienten gut vertragen, in keinem Fall mußte die Untersuchung wegen Nebenwirkungen abgebrochen werden. Die vorliegende Untersuchung zeigt, daß die DSE eine sichere, klinisch nützliche und praktikable nicht-invasive Methode zur Erkennung der KHK und deren Ausmaß ist.
