A Simple Surgical Atrial Septectomy for Complex Congenital Heart Disease

Summary

A simple method of surgical atrial septectomy using a rongeur (Kerrison bone-cutting forceps), which is used in neuro-surgery, is described. The closed rongeur is inserted into the left atrium through the right atrial appendage and the atrial septum. Then it is opened and the edge of the atrial septal defect and/or patent foramen ovale can be hooked by pulling slowly. The rongeur is then tightly closed again and pulled out together with the resected atrial specimen. This technique is safer and simpler than the Blalock-Hanlon technique or atrial septectomy with extra-corporeal circulation for older infants in whom balloon atrial septostomy is difficult.

Zusammenfassung

Es wird eine einfache Methode zur Vorhof-Soptektomie mittels Rongeur (Kerrison-Knochenzange), welcher in der Neurochirurgie Verwendung findet, beschrieben. Die geschlossene Zange wird über das rechte Herzohr durch das Vorhofseptum in den linken Vorhof eingeführt. Anschließend wird sie geöffnet und der Defektrand durch leichten Zug gefaßt. Danach wird die Zange fest geschlossen und zusammen mit dem resezierten Vorhofseptumanteil herausgezogen. Diese Technik ist sicherer und einfacher als die Blalock-Hanlon-Technik oder eine Vor hof-Septektomie unter Verwendung der Herz-Lungen-Maschine bei älteren Kindern, bei denen eine Septektomie mittels Ballon meist nicht zum Erfolg führt.
