Summary
This study was planned to show the effect of retroperfusion and intraaortic balloon pumping (IABP) on myocardial hemodynamic recovery. Twelve dogs entered this study. Half of them received IABP and coronary sinus retroperfusion (CSPR) combination (Group II) and the remaining received IABP alone (Group 1). Left anterior descending artery was occluded for a period of three hours. 15 minutes after occlusion IABP and IABP + CSRP were initiated. The average cardiac output was 1.41 ±0.18 L/min in the group I and 1.72 + 0.24 L/min in the group II (p < 0.03) after 3 hours of occlusion. Mean arterial pressure was 82.1±4.8 mmHg in the group I and 89.7 ± 2.6 mmHg in the group II (p < 0.03).
On the basis of this study it was concluded that CSRP + IABP could be an alternative treatment to IABP alone during the acutely developing ischemia.
Zusammenfassung
In der vorliegenden Studie sollte untersucht werden, welchen Effekt die retrograde Koronarsinus-Reperfusion (CSRP) und die Intraaortale Gegenpulsation (IABP) auf das Verhalten der Myokardfunktion während Ischämie haben. Die Versuche wurden an 12 Hunden durchgeführt. Gruppe I (6 Tiere) wurde mittels IABP behandelt, in Gruppe II (6 Tiere) erfolgte CSRP in Kombination mit IABP. Die Myokardischämie wurde durch einen dreistündigen Verschluß des Ramus interventricularis an terior ausgelöst. 15 Minuten nach Beginn der Koronarokklusion erfolgte der Einsatz der IABP bzw. CSRP und IABP. Nach 3 Stunden Koronarokklusion betrug das Herzzeitvolumen in Gruppe I 1,41 ±0,18 l/min und in Gruppe II 1,72 ± 0,24 l/min (p < 0,03). Der arterielle Mitteldruck betrug in Gruppe I 82,1 ± 4,8 mmHg, in Gruppe II 89,7 ±2,9 mmHg (p < 0,03).
Die Ergebnisse zeigen, daß die Kombination von CSRP und IABP im Vergleich zur alleinigen Anwendung der IABP einen günstigen Einfluß auf die Myokardischämie hat.
