Zusammenfassung
Mit Oxcarbazepin (OXC, Trileptal®) steht in Deutschland jetzt ein weiteres neues Antiepileptikum zur Mono- und Kombinationstherapie von Epilepsien mit fokalen Anfällen mit und ohne sekundäre Generalisierung bei Erwachsenen und bei Kindern ab sechs Jahren zur Verfügung. Es handelt sich um eine dem Carbamazepin (CBZ) ähnliche Substanz, bei der es auf Grund einer Ketogruppe am mittleren Ring des trizyklischen Moleküls aber nicht zu einer oxidativen Metabolisierung und Epoxidbildung kommt. Die wesentliche Wirkung erfolgt über das Monohydroxyderivat (MHD) von OXC, in das eine rasche und weitgehend vollständige Umwandlung erfolgt. Wegen der unterschiedlichen Metabolisierung treten unter OXC deutlich weniger Interaktionen als unter CBZ auf. Darüber hinaus scheinen auch schwerwiegende Nebenwirkungen am blutbildenden System, an der Leber und an der Haut deutlich seltener zu sein als unter CBZ. Die Wirksamkeit von OXC entspricht nach einer Vielzahl von Studien derjenigen der bisherigen Standard-anti-epileptika (neben CBZ insbesondere Phenytoin [PHT] oder Valproat [VPA]), die Verträglichkeit ist im Vergleich zu CBZ und PHT in der Regel besser. Besonders bei Kombinationstherapien z.B. mit VPA ist OXC dem CBZ vorzuziehen, weil VPA zu einem deutlichen Ansteigen von CBZ-Epoxid führt. Nachteile von OXC gegenüber CBZ bestehen im etwas häufigeren Auftreten einer Hyponatriämie, die allerdings meist asymptomatisch bleibt, und den höheren Behandlungskosten. Allerdings ist OXC wesentlich preisgünstiger als die anderen neuen Antiepileptika.
Summary
Oxcarbazepine (OXC, Trileptal®) has now been licensed in Germany as new antiepileptic drug (AED) for mono- and add-on-therapy of epilepsies with focal seizures with and without secondary generalisation in adults and children from the age of six years on. OXC is related to carbama-zepine (CBZ), however, due to a keto group at the middle ring of the tricyclic molecule there is no oxidative metabolism with epoxide formation as with CBZ. The main effect of OXC is due to the monohydroxy derivative (MHD) of OXC, to which OXC is rapidly and almost completely converted in man. Because of the differences in metabolism there are fewer drug interactions with OXC than with CBZ. Furthermore, serious adverse haematotoxic, hepatotoxic and dermatologic side effects seem to occur significantly less often in comparison to CBZ. Being as effective as CBZ or other standard AEDs like phenytoin (PHT) or valproate (VPA), the tolerability of OXC is better than that of CBZ and PHT according to a series of controlled studies. OXC is preferable to CBZ in add-on-therapies e.g. with VPA, which may lead to higher CBZ-epoxide levels. Disadvantages of OXC in comparison to CBZ are the higher rate of hyponatraemia, which, however, remains asymptomatic in the vast majority of cases, and the higher treatment costs. However, OXC is considerably cheaper than the other new AEDs.
