Zusammenfassung
Selegilin (L-Deprenyl) hat eine schwache symptomatische Wirkung beim idiopathischen Parkinson-Syndrom, womit der Einsatz von dopaminergen Substanzen um ca. sechs Monate hinausgezögert werden kann. Umstritten ist die Frage, ob L-Deprenyl neuroprotektiv wirksam ist, wie es verschiedene Tier- und Zellkulturstudien vermuten lassen. Es werden einige klinische Studien vorgestellt, die diese Annahme aber bei aller Vorsicht gerechtfertigt erscheinen lassen. L-Deprenyl gehört meines Erachtens nach wie vor zur Initialtherapie von Patienten mit idiopathischem Parkinson-Syndrom.
Summary
Selegiline (L-deprenyl) shows only moderate symptomatic benefit in patients with idiopathic parkinson's disease. Using L-deprenyl the initiation of levodopa or dopamine agonist can be postponed for about six months. It is rather controversial whether L-deprenyl acts as a neuroprotective substance. There are positive findings in cell culture and in animal models. In this paper we present some international studies which at least indicate that indeed L-deprenyl may be neuroprotective. In our view L-deprenyl should be applied in the initial phase of parkinson's disease.
