Zusammenfassung
150 Patienten wurden mit der Fragestellung eines Normaldruckhydrozephalus innerhalb eines 13-Jahre-Zeitraums untersucht. Als Untersuchungsmethode diente 111-mal der lumbale und 56mal der ventrikuläre Infusionstest in Konstantflußtechnik. Bei 81 Patienten (54%) konnte die Diagnose eines Normaldruckhydrozephalus bestätigt werden; 78 Patienten (96%) unterzogen sich einer Shunt-Operation. Die Graduierung des Normaldruckhydrozephalus nach den Ergebnissen des Infusionstestes in ein Früh- und Spätstadium erlaubt die prognostische Bewertung des Krankheitsverlaufes. Bei kommunizierenden Liquorräumen in den bildgebenden Verfahren der Neuroradiologie bevorzugen wir den lumbalen Infusionstest als patientenfreundliche Untersuchungsmethode. Insbesondere durch die Darstellung des druckabhängigen Abflußwiderstandes R(p) wird gegenüber den international bekannten Verfahren, welche nur einen Widerstandswert pro Infusionstest zum Ergebnis haben, ein erheblicher Informationsgewinn erzielt. So können anhand des Kurvenverlaufes R(p), im Sinne einer spezifischen Patientenkennlinie, Aussagen über das Stabilitätsverhalten und den Typ der vorliegenden Abflußstörung getroffen werden. Der rechnergestützte Infusionstest erlaubt eine sichere Differenzierung zwischen Patienten mit einem Normaldruckhydrozephalus und solchen mit hirnatrophischen Prozessen.
Summary
During a 13-year period we investigated 150 patients suspected for normal pressure hydrocephalus. In 111 patients we used the lumbar and in 56 patients the ventricular infusion test in constant-flow technique. In 81 patients (54%) the diagnosis of a normal pressure hydrocephalus could be confirmed; 78 patients (96%) underwent a shunt operation. A gradvation of the normal pressure hydrocephalus after the results of the infusion test into an early and late stage allows a prognostic evaluation of the course of the disease. While presenting the pressure-dependent resistance to outflow R(p), we could obtain more information in comparison to the internationally used method which provides only one resistance value per infusion test. The course of the curve R(p) can be interpreted as a specific patient curve and provides information on the stability and the type of the existing outflow disturbance. The computer-assisted infusion test enables a definite differentiation between patients suffering from a normal pressure hydrocephalus and those with cerebral atrophy.
