Zusammenfassung
Die Spastik ist ein Syndrom, das sich nach akuten oder chronischen Schädigungen des ZNS einstellt, bei denen es zu Läsionen deszendierender motorischer Bahnen kommt. Da sowohl spinale als auch supraspinale Schädigungen unterschiedlichster Ätiologie Spastik hervorrufen können, allen voran Schlaganfälle, ist die Spastik eines der häufigsten neurologischen Krankheitssyndrome. Die Spastik ist charakterisiert durch den erhöhten, geschwindigkeitsabhängigen Dehnungswiderstand der Skelettmuskulatur, der sich - wie auch die anderen Plus-Symptome der Spastik (gesteigerte Muskeleigenreflexe, Klonus, Pyramidenbahnzeichen, gesteigerte Flexorreflexe) - oft nach einem initial schlaffen Stadium mit ganz unterschiedlichem Verlauf entwickelt. Selten persistieren schlaffe zentrale Paresen. Begünstigende Faktoren für die Persistenz schlaffer Paresen sind hohes Alter und Massenläsionen. Weitere, mit bleibenden schlaffen Paresen assoziierte Faktoren sind ipsilateraler sensomotorischer Neglekt und die Einbeziehung des Nucleus lentiformis in die Läsion. Umgekehrt kommt es zu einer guten motorischen Erholung z.B. der Handfunktion, wenn die Basalganglien oder der laterale Thalamus ausgespart sind. Auch isolierte Kapselläsionen im vorderen oder hinteren Kapselschenkel haben eine günstige Prognose, da die Intaktheit von Teilen der Pyramidenbahn offensichtlich eine Voraussetzung für eine gute funktionelle Erholung ist. Die klinisch nachweisbare spastische Tonuserhöhung beruht auf einer gesteigerten Erregbarkeit der Alpha-Motoneurone. Während selbst-induzierter, natürlicher Bewegungen werden die Dehnungsgeschwindigkeiten, die für die Auslösung dieser gesteigerten Dehnungsreflexe notwendig sind, nicht erreicht. Die typische Bewegungsverlangsamung von Patienten mit Spastik beruht vielmehr auf der assoziierten Parese des Agonisten und auf Transformationen von Muskelfasern spastischer Muskeln, die auch ohne gesteigerte motoneuronale Erregung mit erhöhtem Widerstand auf Dehnungen reagieren.
Summary
Spasticity develops in response to both acute and chronic lesions of descending motor pathways of the CNS. Spinal and supraspinal lesions of various aetiology, including stroke, can cause spasticity. Spasticity is thus one of the most frequent syndromes in neurology. Spasticity is characterised by a velocity-dependent increase in tonic stretch reflexes. Spastic muscle tone can easily be distinguished from other forms of increased muscle tone by its elastic nature and the presence of associated symptoms, such as exaggerated tendon and cutaneous reflexes, clonus, Babinski's sign and autonomic hyperreflexia. In most cases, spasticity is preceded by a period of flaccid paresis of the affected muscles, the duration of which may sometimes be prolonged. Conditions that favour the persistence of these flaccid pareses are: advanced age, a very extended lesion, sensorimotor neglect and involvement of the lentiform nucleus within the lesion. On the other hand, conditions for a favourable recovery of motor function subsequent to flaccid paresis are: intactnessofthe basal ganglia, or of the lateral thalamus. Patients with isolated lesions of the anterior or posterior limb of the internal capsule also have a favourable functional outcome, since the intactness of parts of the corticospinal tract seems to be a prerequisite for good functional recovery. The spastic muscle tone, tested by the clinician by means of fast passive stretchings, is caused by enhanced excitability of alpha motoneurons. However, if the patient voluntarily moves the spastic limb, i.e. during walking, the stretch-induced activity cannot be detected in the antagonists. Thus, other factors are responsible for the slowing-down of the self-paced voluntary movement in patients suffering from spasticity, such as paresis of the agonist and transformations of muscle fibres of spastic muscles. These transformations may lead to increased stretch resistance of spastic muscle fibres (stretch activation).
