Summary
Muscular stiffness is a common, though unspecific symptom in neuromuscular disorders. According to pathogenetic criteria a systematic differential diagnosis is presented with regard to clinical findings in different disorders: Myotonic syndromes, exertional muscle contracture syndrome (Brody's disease), rigid-spine syndrome, metabolic myopathies, neuromyotonia (Isaacs' disease), Satoyoshi's syndrome, tetanus, stiff-man syndrome. In addition muscular stiffness is also a facultative syndrome in polymyalgia rheumatica, fibromyalgia syndrome, hypothyreotic myopathy and steroid-induced myopathy. Therapeutic modalities are presented.
Zusammenfassung
Steifigkeit der Muskulatur ist ein unspezifisches, bei einer Vielzahl neuromuskulärer Erkrankungen vorkommendes Symptom. Es wird eine Einteilung der verschiedenen Syndrome nach pathogenetischen Gesichtspunkten vorgenommen. Es folgt eine Darstellung der wichtigsten klinischen Befunde unter Berücksichtigung differentialdiagnostischer Aspekte von Erkrankungen mit Muskelsteifigkeit als Leitsymptom: myotone Syndrome, prolongierte Muskelrelaxation, Rigid-Spine-Syndrom, metabolische Myopathien, Neuromyotonie, Satoyoshi-Syndrom, Tetanus, Stiff-Man-Syndrom. Die Differentialdiagnose umfaßt ferner Erkrankungen mit Muskelsteifigkeit als fakultatives Begleitsymptom: Polymyalgia rheumatica, Fibromyalgie-Syndrom, hypothyreote Myopathie, Steroidmyopathie. Die Möglichkeiten zur Behandlung von Muskelsteifigkeit werden beschrieben.
