Summary
Thoracic computed tomography (CT) is an essential component in the preoperative Staging
of bronchial carcinomas as is mediastinoscopy (MSC) in cases of mediastinal lymphoma.
It is known that endoscopic ultrasonography (EUS), as a new diagnostic procedure,
can predict lymph-node involvement in cases of tumors in the upper gastrointestinal
tract with an 80% probability.
In a prospective study, we examined whether EUS could be used to ascertain the presence
of mediastinal lymph nodes in cases of bronchial Carcinoma. Since 1990, therefore,
32 patients with operable non-small-cell bronchial Carcinoma have been examined with
an Olympus-Aloka EU-M2 or EU-M3 (frequency 7.5 and 12 MHz) in addition to routine
diagnostics. The graded cross-sections of lymph-node dissections obtained during subsequent
surgery served as evidence as to the true or false prognosis of the lymph-node status.
Endoscopic ultrasonography identifies the presence and estimates the size of subcarinal,
tracheobronchial, paraortal and paraesophageal lymph nodes better than computed tomography.
Lymph nodes lying behind organs containing air (pretracheal lymph nodes) cannot be
identified by ultrasonography.
Lymph-node involvement was correctly identified by EUS in 72% of the cases, and the
specificity was 86%. The poor sensitivity, at 67 %, is explained by the high proportion
(37 %) of patients with anthracosilicosis, as the latter produces the same echo pattern
as malignant infiltration.
In 47% of all the cases, CT showed enlarged mediastinal lymph nodes which were not
actually infiltrated in 67 %. Of these lymph nodes, 33 % could be classified as definitely
free of metastases on the strength of their echo pattern, the rest were inflamed or
really infiltrated by metastases. The diagnostic advantages of EUS, therefore, is
that MSC is rendered unnecessary in cases of both unremarkable and enlarged lymph-node
structures. This means a reduction of MSC of about 30%. In cases of suspected infiltration
on EUS or silicosis, MSC is inevitable.
Zusammenfassung
Zum präoperativen Staging des Bronchialkarzinoms gehört ein thorakales Computertomogramm
(CT) und im Falle mediastinaler Lymphome die Mediastinoskopie (MSK).
Die Endosonographie (EUS) als neues diagnostisches Verfahren vermag bekanntlich bei
Tumoren des oberen Gastrointestinaltraktes mit einer Wahrscheinlichkeit von 80% eine
richtige Vorhersage des Lymphknotenbefalls zu treffen.
In einer prospektiven Studie sollte untersucht werden, ob die EUS auch beim Bronchialkarzinom
zur Beurteilung mediastinaler Lymphknoten von Nutzen sein kann.
Zusätzlich zur Routinediagnostik wurden deshalb seit 1990 mit einem Olympus-Aloka
EU-M2 bzw. EU-M3 (Frequenz 7,5 und 12 MHz) 32 Patienten mit operablem, nicht-kleinzelligem
Bronchialkarzinom untersucht. Die Stufenschnitte der während der anschließenden Operation
entnommenen Lymphknotendissektate dienten als Referenz für die richtige oder falsche
Vorhersage des Lymphknotenstatus.
Besser als im CT gelingt der EUS die Darstellung und Größenbeurteilung der subkarinalen,
tracheobronchialen, paraaortalen und paraösophagealen Lymphknoten. Hinter lufthaltigen
Strukturen liegende Lymphknoten (prätracheale Lymphknoten) können nicht sonographien
werden.
Insgesamt wird der Lymphknotenbefall durch die EUS in 72% richtig vorhergesagt, die
Spezifität beträgt 86%. Die schlechte Sensitivität von 67% wird durch einen hohen
Anteil (37%) von Patienten mit Anthrakosilikose erklärbar, da hierbei das gleiche
Echomuster wie bei der malignen Infiltration vorliegt.
In 47% aller Fälle zeigt das CT vergrößerte Mediastinallymphknoten, die jedoch zu
67% nicht infiltriert waren. 33% dieser Lymphknoten konnten aufgrund ihres Echomusters
als sicher metastasenfrei eingestuft werden, die übrigen waren entzündlich verändert
oder tatsächlich metastatisch infiltriert. Der diagnostische Gewinn durch die EUS
liegt also darin, im Falle unauffälliger Lymphknotenstrukturen, auch bei vergrößerten
Lymphknoten die MSK überflüssig zu machen. Die Einsparung beträgt ca. 30%. Bei endosonographischem
Verdacht auf Infiltration oder bei Silikose wird nach wie vor auf die MSK nicht zu
verzichten sein.
