Summary
The plasma levels of protein C, AT III, the perioperative administration of fresh frozen plasma (FFP) and AT III concentrate were compared under the use of various drugs in a randomized, prospective double-blind study in 40 patients in whom an aortocoronary bypass operation was carried out. We formed four groups of ten patients: group A served as a control group, group B received a prostacyclin (PGI2) infusion of 10 or 20ng/kg/min, group C high-dose aprotinin Substitution, and group D was treated with a combination of prostacyclin and aprotinin. After an initial short-term rise in the inhibitors protein C and AT III, there was a fall in all groups in the further course of extracorporeal circulation. The initial preoperative values were reached again on the morning of the first postoperative day. This indicates a raised turnover and in association with this a raised rate of elimination of these factors caused by an increased thrombin activation during the extracorporeal circulation which cannot be prevented by the usual heparinization. Whereas prostacyclin had no effect on our results mediated by thrombocytic mechanisms, use of aprotinin led to a significant saving in the requirement for perioperative fresh frozen plasma and AT III Substitution therapy. A clinical advantage of prostacyclin and aprotinin combination was not observed. In view of our results (individual analyses were mainly in the normal range), we consider that AT III and fresh frozen plasma should not be substituted routinely during or after extracorporeal circulation.
Zusammenfassung
Im Rahmen einer randomisierten, prospektiven Doppelblindstudie wurden bei 40 Patienten, die zu einer aorto-koronaren Bypassoperation anstanden, die Plasmaspiegel von Protein C, AT III, der perioperative Einsatz von Fresh-Frozen-Plasma (FFP) und AT III-Konzentrat sowie die Auswirkungen verschiedener Medikamente auf diese Verläufe gemessen. Dazu bildeten wir 4 Gruppen mit je 10 Patienten: Gruppe A diente als Kontrollgruppe, Gruppe B erhielt eine Prostacyclin (PGI2)-Infusion von 10 bzw. 20ng/kg/min, Gruppe C wurde hochdosiert Aprotinin substituiert und Gruppe D wurde mit einer Kombination von Prostacyclin und Aprotinin behandelt. Nach einem initialen kurzzeitigen Anstieg der Inhibitoren Protein C und AT III kam es im weiteren Verlauf der extrakorporalen Zirkulation in allen Gruppen zu einem Abfall, bis zum Morgen des 1. postoperativen Tages waren dann die präoperativen Ausgangswerte wieder erreicht. Dies spricht für einen gesteigerten Umsatz und einer damit verbundenen erhöhten Eliminationsrate dieser Faktoren, ausgelöst durch eine vermehrte Thrombinaktivierung während der extrakorporalen Zirkulation, die durch die übliche Heparinisierung nicht verhindert werden kann. Während Prostacyclin über thrombozytäre Mechanismen keinen Einfluß auf unsere Ergebnisse hatte, führte die Anwendung von Aprotinin zu einer signifikanten Einsparung bei der perioperativen Fresh-Frozen-Plasma- und AT III-Substitutionstherapie. Ein klinischer Vorteil durch die Kombination von Prostacyclin und Aprotinin war nicht zu beobachten. Aufgrund unserer Ergebnisse (Einzelanalysen lagen überwiegend im Normalbereich) sprechen wir uns gegen eine routinemäßige Substitution von AT III und Fresh-Frozen-Plasma während oder nach der extrakorporalen Zirkulation aus.
