Summary
The successful resection of an abdominal aortic aneurysm is presented in a patient who had undergone kidney transplantation 4 years previously. Because the transplanted kidney is more sensitive to ischemia than a normal one, a femorofemoral bypass with a pump oxygenator was used for perfusion of the transplanted kidney during crossclamping. During the clamping time of 40 minutes kidney perfusion was maintained with a perfusion pressure of 60 to 80mmHg and the flow was 600 to 1000ml/min. A collagen-seeded Dacron graft (diameter: 18mm, length: 12mm) was interposed. The postoperative course was uncomplicated. We believe that performing the femoro-femoral bypass with a pump oxygenator is an effective and simple method for kidney protection in such operations.
Zusammenfassung
Es wird über einen 51jährigen Patienten berichtet, bei dem 4 Jahre nach Nierentransplantation ein infrarenales Bauchaortenaneurysma operiert wurde. Da die Transplantatniere in besonderem Maße ischämiegefährdet ist, benützten wir einen femoro-femoralen Bypass mittels HLM als Ischämieschutz. Während der 40minütigen Abklemmzeit wurde die Transplantatniere mit Druckwerten zwischen 60 und 80mmHg perfundiert, der dazu erforderliche Fluß lag zwischen 600 und 1000 ml/min. Eine Kollagenbeschichtete Dacron-Rohrprothese (18 mm × 12 cm) wurde eingesetzt. Der postoperative Verlauf war unkompliziert. Wir halten den femoro-femoralen Shunt mittels HLM für eine wirksame und einfache Methode zum Ischämieschutz der Transplantatniere bei derartigen Operationen.
