Summary
This paper reports on 1097 carotid artery operations (651 in stage I and 446 in stage II of cerebrovascular insufficiency) carried out from 1970 to 1988. The indication for surgery was based on the following criteria: hemodynamic efficacy of the stenosis, risk of embolism of the vascular lesion, causal connection between the extracranial stenosis and the neurological deficit, and exclusion of a serious concomitant internal disease. As operation technique, open thrombendarterectomy using an intraluminal shunt was applied. The arteriotomy was closed by means of a patch. When there was a simultaneous elongation of the internal carotid artery (kinking), the internal was shortened with reinsertion into the common carotid artery. In stage I the surgical mortality was 0.9%, and the surgical morbidity (cerebrovascular deficit of ischemic origin) was 1.2%; in stage II the corresponding figures were 1.2% and 2.4%respectively. The period of follow-up observation comprised three to 207 months (up to 17 years) with an average value of 70 months for patients of stage I and 67 months for patients of stage II. Patients of stage I suffered a stroke in 1.4% (n = 9) and patients in stage II suffered a stroke in 2.4% (n = 10) of cases i.e. annually 0.2% in stage I and 0.3% in stage II. There have been various reports on the favorable spontaneous course of patients with carotid stenoses treated nonsurgically. A risk of stroke of 1 % to 3 % was reported for stage I and a risk of stroke of 3 % to 4% was reported for stage II. Nevertheless, the author regards surgical treatment of critical carotid stenosis as the method of choice, since the incidonce of stroke has been reduced tenfold (0.2% and 0.3% respectively) in the present patient population.

Es wird über 1097 Karotisoperationen berichtet, die von 1970-1988 ausgeführt wurden. 651 Patienten befanden sich im Stadium I und 446 im Stadium II. Der Operationsindikation lagen die folgenden Kriterien zugrunde: hämodynamische Wirksamkeit der Stenose, embolisches Risiko der Gefäßläsion, ursächlicher Zusammenhang zwischen der extrakraniellen Stenose und dem neurologischen Defizit sowie Ausschluß einer gravierenden internistischen Begleiterkrankung. Operationstechnisch wurde die offene Ausschälplastik (TEA) unter Verwendung eines intraluminalen Shunts durchgeführt. Die Arteriotomie wurde mit Hilfe eines Streifentransplantates verschlossen. Wenn gleichzeitig eine Elongation der A. carotis interna (Kinking) bestand, wurde eine Kürzung der Carotis interna mit Reinsertion in die A. carotis communis vorgenommen. Im Stadium I betrug die Operationsletalität 0,9%, die Operationsmorbidität (ischämisch bedingtes zerebrovaskuläres Defizit) 1,2%; im Stadium II lauten die entsprechenden Zahlen: 1,2 bzw. 2,4%. Der Nachbeobachtungszeitraum umfaßte 3-307 Monate (bis zu 17 Jahren) mit einem Durchschnittswert von 70 Monaten für Patienten des Stadiums I und 67 Monaten für Patienten des Stadiums II. Patienten des Stadiums I erlitten in 1,4% (n = 9) und solche des Stadiums II in 2,4% (n = 10) einen Schlaganfall, d.h. die jährliche Schlaganfallquote betrug 0,2% (Stadium 1) bzw. 0,3% (Stadium II). Trotz verschiedener Berichte über den günstigen Spontanverlauf nicht-operativ behandelter Patienten mit Karotisstenosen - für Stadium I wurde ein Schlaganfallrisiko von 1 -3% und im Stadium II eines von 3-4% angegeben - sehen die Autoren die operative Behandlung einer kritischen Karotisstenose als die Methode der Wahl an, da im vorliegenden Krankengut die Schlaganfallinzidenz mit 0,2 bzw. 0,3% um das Zehnfache reduziert ist.
