Summary
Septic multi-organ failure represents a common cause for operative mortality following open-heart surgery. One major reason might be the depression of cell-mediated immunity. The purpose of this prospective randomized trial was to quantify and specify the effects of open-heart surgery on cell-mediated immune mechanisms. In addition, the immunorestorative potential of a combined immunomodulatory therapy with the cyclooxygenaso inhibitor indomethacine and the thymomimetic substance Thymopentin versus single drug administration of indomethacine was investigated. Twenty patients were given indomethacine for the first 5 postoperative days (group A). Another 20 patients also received Thymopentin porioperatively and on the second and fourth postoperative days (group B), while 20 patients underwent conventional therapy (group C). Cell-mediated immune response was quantified in vitro by measuring CD3+ T-lymphocytes and their subsets, CD4+ T-helpor and CD8+ T-supprossor cells. Lymphocyte responsiveness to a specific (Antigen-Cocktail) and non-specific mitogen (phytohemagglutinin) provided information about the quality of cell-mediated immune response. On the first postoperative day CD3+ T-lymphocyte counts and Antigen-Cocktail-induced lymphocyte proliferation decreased significantly in all groups. The number of CD4+ T-Helper cells fell significantly only in groups A and C, while the decrease in group B was not statistically significant; the same applied to phytohemagglutinin-induced lymphocyte response. The CD4+/CD8+ ratio was significantly depressed only in group C, decreased slightly in group A and did not change as compared to baseline values in group B. All investigated parameters remained significantly depressed until the seventh postoperative day in group C. In group A CD3+ T-lymphocytes, CD4+ T-helper cells and CD4+/CD8+ ratio almost returned to baseline on the seventh postoperative day, while lymphocyte response to specific and non-specific mitogen remained significantly suppressed. In contrast, all investigated parameters even increased significantly at the end of the first postoperative week in group B. It is concluded from these data that cell-mediated immunity is significantly depressed for at least one week following open-heart surgery. Single drug administration of indomethacine could not completely restore cell-mediated immunity. Only simultaneous administration of indomethacine and Thymopentin offered sufficent protection of the cell-mediated immune response.
Zusammenfassung
Das septische Mehrorganversagen stellt einen Hauptgrund für die operative Sterblichkeit herzchirurgischer Patienten dar. Eine Ursache könnte die Suppression der zellvermittelten Immunität darstellen. Ziel der prospektiven, randomisierten Studie war es daher, die Effekte der Herzchirurgie auf die zellvermittelte Immunität sowohl quantitativ als auch qualitativ zu untersuchen. Zusätzlich sollte untersucht werden, ob eine kombinierte immunmodulatorische Medikation in Form des Cyclooxygenaseinhibitors Indometacin und der thymomimetischen Substanz Thymopentin einer Einzeltherapie mit Indometacin hinsichtlich der Wiederherstellung der zellvermittelten Immunität überlegen ist. 20 Patienten erhielten perioperativ für 5 Tage Indometacin (Gruppe A). Weitere 20 Patienten erhielten zusätzlich unmittelbar präoperativ und am 2. und 4. postoperativen Tag Thymopentin (Gruppe B), während bei 20 Patienten keine zusätzliche Medikation verabreicht wurde (Gruppe C). Die zellvermittelte Immunität wurde quantitativ in vitro durch die Messung der CD3+ T-Lymphozyten und ihrer Subpopulationen, der CD4+ T-Helferzellen und der CD8+ T-Suppressorzellen bestimmt. Die Lymphozytenproliforation nach spezifischer (Antigen-Cocktail) und unspezifischer (Phytohämagglutinin) mitogener Stimulation diente zur qualitativen Bourteilung der zellvermittelten Immunantwort. Am 1. postoperativen Tag kam es zu einor signifikanten Erniedrigung sowohl der CD3+ T-Lymphozyten als auch der Antigen-Cocktail-induzierten Lymphozytenproliferation in allen Gruppen. Die Zahl der CD4+ T-Helferzellen nahm wie die Phytohämagglutinin-induzierte Proliferation nur in den Gruppen A und C signifikant ab. Das CD4+/CD8+-Verhältnis war nur in der Gruppe C signifikant erniedrigt, nahm in der Gruppe A ab, ohne Signifikanzniveau zu erreichen, und zeigte in der Gruppe B keine Veränderungen gegenüber den Ausgangswerten. Alle untersuchten Parameter blieben in der Gruppe C bis zum 7. postoperativen Tag signifikant erniedrigt. In der Gruppe A erreichten die Werte bei den CD3+ T-Lymphozyten, den CD4+ T-Helferzellen und dem CD4+/CD8+-Verhältnis am 7. postoperativen Tag das präoperative Niveau, während die spezifisch und unspezifisch induzierte Lymphozytenproliferation signifikant supprimiert blieb. Im Gegensatz dazu waren alle untersuchten Parameter in der Gruppe B gegenüber den Ausgangswerten am 7. postoperativen Tag signifikant erhöht. Die Resultate der Studie zeigen, daß die zellvermittelte Immunität nach herzehirurgischen Eingriffen für zumindest 1 Woche signifikant supprimiert ist. Die Einzelmedikation mit Indometacin konnte die zellvermittelte Immunität nicht vollständig wiederherstellen. Erst die gleichzeitige Gabe von Indometacin und Thymopentin ermöglichte einen ausreichenden Schutz des zellvermittelten Immunsystems.
