Summary
The prosent report describes the case of a 57-year-old patient with malignant fibrous
histiocytoma of the left atrium operated on for a suspected atrial myxoma. Surgery
was porformed as radically as possible and includod resection of the atrial wall.
Clinical investigation and echocardiography performed 29 months after surgery revealed
no signs of tumor recurrence. Diagnostic procedures, topography, pathohistology as
well as options for therapy are discussed with regard to the latest literature. A
radical surgical approach represents the only effective therapeutic measure. Owing
to a lack of sufficient experience, there is no additional therapeutic recommendation
on hand. Neither radiation nor chemotherapeutic treatment concepts have proved efficient.
Echocardiography represents the best examination procedure for both diagnosis and
follow-up in patients with cardiac tumor.
Zusammenfassung
Bei einem 57 Jahre alten Patienten wurde unter dem Verdacht eines Vorhofmyxoms ein
malignes fibröses Histioyztom des linken Vorhofs operiert. Die Resektion wurde so
radikal wie möglich unter Einschluß der Vorhofwand ausgeführt. Die klinische Untersuchung
und Echokardiographie zeigte 29 Monate postoperativ keine Hinweise auf ein Tumorrezidiv.
Diagnostische Maßnahmen, Topographie, Morphologie und therapeutische Möglichkeiten
werden unter Berücksichtigung der neueren Literatur diskutiert. Die radikale chirurgische
Resektion stellt die einzige effektive therapeutische Maßnahme dar. Aufgrund mangelnder
Erfahrung steht keine zusätzliche Therapieempfehlung zur Verfügung. Weder strahlen-
noch chemotherapeutische Konzepte haben sich bisher als wirksam erwiesen. Die Echokardiographie
ist die beste Untersuchungsmethode für Diagnostik und Nachuntersuchung bei Patienten
mit Herztumoren.
