Summary
The anatomy of valsalva's sinus is such that it decreases the mechanical stress on the leaflets of the aortic valve. Artificial stents impair the hemodynamic performance of an aortic bioprosthesis and lead to unphysiological functional stresses and may also shorten their durability. Therefore a semi-supported xenograft was developed which can be attached with an onesuture row in subcoronary position. This facilitates surgery considerably. Moreover, the commissures can be attached directly to the patient's aortic wall. The semi-supported valve was successfully implanted in a 69 years old patient with a combined aortic valve lesion. Postoperative invasive investigations, including angiography and videodensitometry, showed a normal performance of the valve and a normal anatomy of the aortic root.
Zusammenfassung
Die menschliche Aortenwurzel stellt mit den Sinus valsalvae und den Taschenklappen eine funktionelle Einheit dar. Bioprothesen mit an Halterungen befestigten Kommissuren stören den Öffnungs- und Schließmechanismus der Aortenklappe erheblich, ihre unphysiologische Belastung führt u. a. zu einer Minderung ihrer Haltbarkeit. Um einen normalen Bewegungsablauf der implantierten glutaraldehyd-pra pörierten porcinen Aortenklappe zu erhalten, wurde eine teilunterstützte Aortenklappe hergestellt, die mit einer Nahtreihe in subkoronarer Position befestigt und deren flexible Kommissuren direkt an die Aortenwand des Patienten angenäht wird. Eine solche teilunterstützte Bioprothese wurde Anfang Juni 1989 einem 69jährigen Patienten in subkoronare Position implantiert. Die postoperativen invasiven Untersuchungen, einschließlich Angiographie und Videodensitometrie, ergaben weder einen Druckgradienten noch einen Volumenreflux in die linke Kammer. Der angiographische Aspekt der Aortenwurzel mit der teilunterstützten implantierten Bioprothese erscheint in Systole und Diastole normal.
