Zusammenfassung
Der Beginn eines „Tethered-Cord” Syndroms im Erwachsenenalter ist selten, da es aufgrund der Hauterscheinungen im Lumbalbereich (Hypertrichose, lumbales Lipom, Nävus) meist bereits im Kindesalter diagnostiziert wird. Es ist in der Regel mit einem Tiefstand des Rückenmarkes unterhalb des physiologischen Niveau L 1/2, d.h. mit einer Aszensionsstörung des Rückenmarkes und einem verdickten Filum terminale vergesellschaftet. Wir berichten über einen Fall einer 44jährigen Patientin mit „Tethered-Cord” Syndrom ohne die sichtbaren Zeichen der dysraphischen Fehlbildung und ohne Aszensionsstörung des Rückenmarkes.
Bei der Operation wurde das verdickte Filum terminale exstirpiert und das Rückenmark von der Zugbelastung befreit. Die Blasenentleerungsstörungen sowie die Schmerzsymptomatik besserten sich postoperativ. Ein zusätzlich vorhandener Bandscheibenvorfall verdeckte zunächst die Grunderkrankung.
Die radiologische Diagnostik wird dargestellt und der Fall wird anhand der Literatur diskutiert.
Summary
The „tethered cord” syndrome is rarely detected in adults, because it is usually diagnosed in childhood on account of the skin changes in the lumbar region (hypertrichosis, lumbar lipoma, nevus). It is usually associated with a thickened filum terminale and spinal cord terminating beneath the physiological level L 1/2, i. e.: with disturbance to ascension of the cord. We describe the case of a 44-year olf female patient with “tethered cord” syndrome but without the visible signs of dysraphic malformation and without disturbed ascension of the spinal cord.
At operation the thickened filum terminale was extirpated, freeing the spinal cord from the tension. The disturbances to urinary continence, and also the symptomatic pain, improved post-operatively. A prolapsed lumbar disc, also present, at first hindered diagnosis of the basic disease.
We present the radiological diagnostic work up and discuss the case with reference to the literature.
