Zusammenfassung
Das früher fast nur von Hirnsektionen bekannte, seltene intrakranielle Lipom kann bei sorgfältiger Beachtung auch kleiner Befunde computertomographisch in knapp 0,1% nachgewiesen werden. Die bevorzugt in der Mittellinie und paramedian lokalisierte Gewebsveränderung ist nach pathologischen Kriterien als Hamartom zu klassifizieren, somit als Gewebsfehldifferenzierung von primär nicht neoplastischem Charakter. Ätiologisch ist ein Zusammenhang mit dysraphischen Störungen wahrscheinlich. In 7 der 9 hier beschriebenen Fälle ist der Balken betroffen oder beteiligt, wobei sich 5mal zusätzliche, knötchenförmige Anteile im Plexus der Seitenventrikel uni- oder bilateral nachweisen lassen. Die bekannte Häufung von epileptischen Anfällen und psychischen Auffälligkeiten beim lipomatösen Hamartom des Balkens wird im vorliegenden Krankengut in 3 von 7 Fällen (43%) bestätigt; ein sogenanntes Diskonnektionssyndrom konnte in einigen daraufhin untersuchten Fällen jedoch nicht nachgewiesen werden. Bei einem 3jährigen Mädchen war ein kleines lipomatöses Hamartom der Vierhügelzisterne mit einem frontalen zystischen Astroblastom kombiniert. Pathogenese und klinische Wertigkeit der komplexen Malformation werden anhand der erhobenen Befunde diskutiert.

Clinical and computerized tomographic findings in 9 cases
By computed tomography (CT) intracranial lipomas can be detected in nearly 0,1% of the cases, including very small lesions. The tumorlike malformation should be classified as a hamartoma, e. g. as a maldifferentiation of primarily non-neoplastic tissue. The preferential location is the midline region; a pathogenetic relation to dysraphic disorders is possible. 7 of the 9 cases presented have a lipomatous hamartoma of the corpus callosum or show involvement of this region; in 5 cases additional lipomatous nodules of the ventricular chorioid plexus could be demonstrated uni- or bilaterally. In agreement with the literature, epilepsy and mental disturbances are frequent (43%) also in our cases of lipoma of the corpus callosum. A callosal disconnection syndrome could not be demonstrated in the two patients thoroughly examined. A 3-year-old girl showed a small lipomatous hamartoma of the ambient cistern in combination with a large cystic frontal astroblastoma. Clinical and CT-findings are discussed with regard to pathogenesis and pathological significance of this complex malformation.
