Zusammenfassung
Die hysteroskopische Endometrium-Resektion ist eine relativ neue und vielfach noch kontrovers diskutierte Operationsmethode zur Behandlung von dysfunktionellen uterinen Blutungen. Material und Methode: Im Rahmen einer retrospektiven Analyse berichten wir über unsere operativen Erfahrungen mit 500 transzervikalen Endometriumresektionen (TCRE). 67% der Operationen wurden in Allgemeinanästhesie und 33% unter Sedierung in Lokalanästhesie durchgeführt. Das Durchschnittsalter der Patientinnen betrug 42,7 Jahre und 88.4% waren mehrgebärend. Mehr als 85% der Patientinnen hatten therapieresistente Blutungsstörungen und etwa Ÿ aller Fälle wurden präoperativ mit Danazol vorbehandelt. Ergebnisse: In 99,8% aller Fälle konnte die Operation erfolgreich beendet werden, wobei das Endometrium meist komplett (95,2%) reseziert wurde. Die Operationszeit betrug durchschnittlich 33,3 ±14,2 min und war in Fällen mit zusätzlicher Myomresektion (22,4%) und bei Patientinnen ohne Endometrium-Präparation (27%) signifikant länger. Die durchschnittliche Flüssigkeitsabsorption betrug 584 ml. Operationsdauer, Endometrium-Präparation, Uterusgröße sowie eine zusätzliche Myomresektion und die Tubendurchgängigkeit erwiesen sich als entscheidende Faktoren für ein höheres Flüssigkeitsdefizit. Ein „fluid-overload“-Syndrom und Uterusperforationen sind bei je 10 Patientinnen aufgetreten und in zwei Fällen mußten stärkere Blutungen mit einem Foley-Katheter tamponiert werden. Die postoperative Morbidität lag bei 4,4% und 85% der Patientinnen konnten innerhalb eines Tages entlassen werden. Schlußfolgerung: Die TCRE ist ein relativ kurzer Eingriff mit einer geringen Morbidität. Sorgfältige Patientenauswahl, exakte Indikationsstellung und vor allem Kenntnis und Erfahrung des Operateurs sind die wichtigsten Grundvoraussetzungen einer erfolgreichen Operation.
Abstract
Purpose: As with all new techniques in gynaecological endoscopy, perioperative parameters such as feasibility, safety, and morbidity, can be assessed only from large series. In this observational study, we report on our experiences and short-term outcome of 500 consecutive patients who underwent transcervical resection of the endometrium (TCRE) in our unit. Material and Methods: 337 procedures were performed under general anaesthesia (67.4%) and 163 under sedation plus local anaesthesia (32.6%). The women were of mean age 42.7 years, and 88.4% were parous. The main indication for surgery was abnormal menstruation (99.4%); in 85% of the cases this was refractory to any prior treatment. Almost 73% of the patients were given danazol preoperatively to suppress endometrial proliferation. Results: Hysteroscopic surgery was completed successfully in 99.8%, and complete and partial resections were performed in 95.2% and 4.6%, respectively. The average operative time was 33.3 ± 14.2 min, shown to be significantly longer in patients who underwent simultaneous myomectomy (22.4%), and those without endometrial preparation (27%). The mean intraoperative fluid balance was 584 ml, influenced by the duration of surgery, endometrial preparation, uterine size, the need for hysteroscopic myomectomy, and tubal patency. Clinical complications such as significant fluid overload and uterine perforation occurred in 10 patients each (4%); two subjects required uterine tamponade to control bleeding (0.4%). Morbidity in our series was 4.4% overall, and 85% of patients were discharged from hospital within 24 hours. Conclusions: TCRE is a relatively safe and short operation, associated with a low morbidity and rapid postoperative recovery. Careful patient selection, extensive training and surgical skill are required to minimise complications. A minimally invasive alternative to hysterectomy, TCRE offers major clinical benefits in the management of women with dysfunctional uterine bleeding.
