Zusammenfassung
Die p-53-Genmutation mit der Folge der Akkumulation mutanter p-53-Proteinformen im Tumorzellkern von Mammakarzinomen führt zum Verlust der negativen Regulation des als Tumorsuppressor zu bezeichnenden p-53-Gens. Mutante p 53-Proteinformen können im Tumorzellkern immunhistochemisch am Paraffinschnitt nachgewiesen werden (Klon DO 1, Dianova). An 482 primären Mammakarzinomen wurde der p-53-Proteinnachweis durchgeführt, wobei in 21,6% der Fälle die Immunhistochemie des p-53-Proteins als positiv beurteilt wurde. In 14,3 % der Mammakarzinome fanden sich in weniger als 10% der Tumorzellen positive p-53-Färbungen. In 64,1 % der Mammakarzinome war die Immunhistochemie des p-53-Proteins negativ. In Abhängigkeit vom immunhistochemischen Färbeergebnis wurde der Krankheitsverlauf primärer Mammakarzinome retrospektiv ausgewertet: Hierbei ergab sich für p-53-positive Mammakarzinome eine signifikant ungünstigere Prognose im Vergleich zu den p-53-negativen oder schwach-positiven Primärtumoren. Die p-53-Akkumulation im Tumorzellkern ist korreliert mit dem negativen Östrogen- und Progesteronrezeptorstatus sowie der Proliferationsaktivität des Mammakarzinoms. Die p-53-Akkumulation ist unabhängig vom Tumorstadium, dem histologischen Grading, dem Menopausenstatus und der c-erb-B-2-Überexpression. Im Subkollektiv nodal-negativer Mammakarzinompatientinnen weisen p-53-positive Mammakarzinome ein ungünstigeres krankheitsfreies Intervall sowie ein kürzeres Gesamtüberleben auf. Die Unterschiede sind jedoch im Vergleich zu p-53-negativen Mammakarzinomen bei einer mittleren Nachbeobachtungszeit von 48 Monaten nicht signifikant. Die Cox-Analyse der Aussagekraft der untersuchten Prognoseparameter weist auf die Bedeutung der p-53-Proteinakkumulation als unabhängiger Prognoseparameter hin. Die universelle Bedeutung der p-53-Genmutation für die Karzinogenese humaner Tumoren macht das immunhistochemische Nachweisverfahren interessant für die Untersuchung anderer gynäkologischer Karzinome und die Evaluierung der biologischen Eigenschaft.
Abstract
The occurrence of the p 53 gene mutation in breast carcinoma tumour cells, leads to the accumulation of mutant p 53 protein types, whose consequence is the loss of the negative regulation normally exercised by the p 53 gene, which is considered to act as a tumour suppressor. It is possible to demonstrate the presence of mutant p 53 protein types in tumour cell nuclei by applying immunohistochemical procedures to paraffin sections (Clon DO 1, Dianova). We tested 482 primary breast carcinomas for the presence of these proteins, and positive immunohistochemical findings for mutant p 53 proteins were recorded in 21.6% of the cases. In another 14.3 % of these breast carcinomas, less than 10% of the tumour cells exhibited positive staining. In the other 64.1 % of cases, the immunohistochemical findings for p 53 proteins were entirely negative. Independent of the immunohistochemical staining results, we performed a retrospective analysis of the disease course of this group of primary breast carcinomas: it emerged, that p-53-positive breast carcinomas had a significantly less favourable prognosis as compared to primary tumours, which were negative or weakly positive for this protein group. The accumulation of p 53 proteins in tumour cell nuclei is correlated with negative oestrogen- and progesterone-receptor status, as well as with the degree of proliferation exhibited by the breast carcinoma. Such accumulation is, in contrast, unaffected by the tumour stage, its histological grading, menopausal status, and the overexpression of c-erb B2. In a subgroup of patients with node-negative breast carcinomas, p-53-positive tumours produced less favourable findings with regard to both the disease-free interval and overall survival. However, over the mean follow-up period of 48 months, the difference between these findings and those for p-53-negative breast carcinomas did not attain statistical significance. Cox analyses to assess the predictive value of the tested prognosis parameters indicated that the factor p-53 protein accumulation can be regarded as an independent prognosis parameter. The general significance of p 53 gene mutation in respect of the carcinogenesis of human tumours means, that the immunohistochemical procedure for the demonstration of the presence of p 53 proteins is of interest for the investigation of other gynaecological carcinomas and the assessment of their biological characteristics.
