Zusammenfassung
Taxol ist ein pflanzliches Zytostatikum mit einer komplexen Struktur und einem besonderen Wirkungsmechanismus. Die Substanz ist aufgrund des geringen Gehaltes in der Rinde der Pazifischen Eibe, aus der Taxol extrahiert wird, bisher nur in begrenztem Maße verfügbar. Die chemische Struktur von Taxol stand bisher der Synthese großer Mengen dieses Zytostatikums entgegen. Analogsubstanzen lassen sich aus den Nadeln der Eibe gewinnen, es sind jedoch effizientere Methoden zur großtechnischen Herstellung dieses Medikaments erforderlich. In der vorliegenden Arbeit werden Ergebnisse präklinischer und klinischer Studien bei Patientinnen mit Ovarial- und Mammakarzinom zusammengfaßt. In Phase-I-Studien konnte das Nebenwirkungsprofil von Taxol aufgezeigt werden, wobei die Neutropenie in den meisten Untersuchungen dosislimitierend war. Die Wirksamkeit von Taxol bei Patientinnen mit Ovarialkarzinom konnte zunächst in Phase-I-Studien demonstriert werden. Diese Ergebnisse führten zu Phase-II-Studien, in denen sich Ansprechraten von 20-36 % bei Patientinnen mit rezidiviertem oder therapierefraktärem Ovarialkarzinom ergaben. Zur Zeit wird in Phase-III-Studien die Wirksamkeit von Taxol in Kombination mit Cisplatin im Vergleich zu dem klassischen Regime Cisplatin/Cyclophosphamid geprüft. Weitere Studien testen Taxol in Kombination mit G-CSF und Cisplatin. Ergänzend zur intravenösen Taxoltherapie existieren erste Erfahrungen mit der intraperitonealen Taxolbehandlung. Die zytostatische Wirksamkeit von Taxolbestätigte sich auch in der Therapie von Patientinnen mit metastasiertem Mammakarzinom mit Remissionsraten von 56-62 %. Taxol ist ein wichtiges neues Zytostatikum zur Therapie des Ovarial- und Mammakarzinoms. Die besondere Wirkungsweise und die relativ geringe Toxizität machen Taxol zu einer attraktiven Substanz, insbesondere in Kombination mit bisher etablierten Zytostatika. Der Stellenwert von Taxol im therapeutischen Konzept des Ovarial- und Mammakarzinoms wird derzeit durch weitere Studien geprüft. In absehbarer Zeit ist mit den Ergebnissen multizentrischer Studien zu rechnen. Der breite Einsatz dieser Substanz wird von der Entwicklung einer praktikablen großtechnischen Methode zur Synthese dieses neuen Zytostatikums abhängen.
Abstract
Taxol is a cytostatic agent of plant origin with a complex structure and a special mode of action. Owing to the small content of this substance in the bark of the Pacific yew, from which Taxol is derived, it has only been available on a limited scale. Hitherto, Taxol's chemical structure has precluded the synthesis of large quantities of this cytostatic agent. Analogue substances can be obtained from the needles of the yew, although more efficient methods are needed for the manufacture of this medicine on a commercial scale. The present paper summarises the results of preclinical and clinical studies of patients suffering from ovarian and breast cancer. In phase I studies, it was possible to delineate the side effect profile of Taxol, neutropenia being doselimiting in most investigations. The efficacy of Taxol on patients with ovarian cancer was initially demonstrated in phase I studies. These results led to phase II studies, in which response rates of 20-36% were obtained on patients with relapsing or therapy-refractory ovarian carcinomas. In phase III studies, the efficacy of Taxol combined with cisplatinum is currently being compared with the classical regimen of cisplatinum /cyclophosphamide. Further studies are under way testing Taxol in combination with G-CSF and cisplatinum. As a supplement to intravenous Taxol therapy, initial experience with intraperitoneal Taxol treatment is now available. Taxol's cytostatic efficacy has also been confirmed in the treatment of patients suffering from metastatic breast cancer with remission rates of 56-62%. Taxol is an important new cytostatic agent for the treatment of ovarian and breast cancer. Its special mode of action and its relatively low toxicity make Taxol an attractive substance, particularly if combined with established cytostatic agents. The value of taxol in the therapeutic concept for ovarian and breast cancer is currently investigated in further studies. Results from multicentric studies are anticipated in the foreseeable future. The broad application of this substance will depend on the development of a practicable commercial-scale method for synthesizing this new cytostatic agent.
